Transbronchial needle aspiration "by the books"

Abstract: BACKGROUND:Training for advanced bronchoscopic procedures is acquired during the interventional pulmonology (IP) Fellowship. Unfortunately a number of such programs are small, limiting dissemination of formal training.OBJECTIVE:We studied success of conventional transbronchial needle aspiration (C-TBNA) in the hands of physicians without formal IP training.METHODS:A technique of C-TBNA was learned solely from the literature, videos and practicing on inanimate models at “Hands-On” courses. Conventional TBNA wit… Show more

“…Out of seven studies that assessed this outcome in an unselected population, six reported higher sensitivity for lymph nodes ≥2 cm in the short axis with a linear relationship with increasing size, when evaluated [5,9,10,12,13,14]. Similar results were obtained in four out of seven studies, involving patients with suspected/known lung cancer [20,21,22,25].…”

“…Conventional transbronchial needle aspiration is an established method for diagnosing mediastinal pathologies. We have been performing this procedure at our institution since 2009, with a success rate similar to previous published reports . In recent years, there has been increasing interest in endobronchial ultrasound‐guided transbronchial needle aspiration as evident by the increasing number of publications, international seminars and hands‐on courses .…”

“…However, these methods are not widely practiced, often because of a perceived low diagnostic yield or risk of pneumothorax or hemoptysis. [1821] More invasive tools such as surgical mediastinoscopy or thoracoscopy may pose significant risk, as well as cost, to the patient, requiring general anesthesia and operating room use. [22]…”
