Transbronchial Cryobiopsy in Interstitial Lung Disease

Abstract: A transbronchial lung cryobiopsy using a flexible cryoprobe has a good diagnostic yield and might be an alternative to consider in cases of diffuse ILD in which a histologic sample is required for diagnosis. This technique could avoid a large number of surgical biopsies.

“…Several pilot studies have described the use of a smaller 1.1-mm cryoprobe, which can be pulled through a sheath placed in the working channel of the bronchoscope, allowing the bronchoscope to stay in place during cryoprobe retrieval and decreasing the concerns for uncontrolled endobronchial bleeding [90,91]. However, it is unclear whether these smaller cryobiopsies will result in diagnostic yields similar to those described with larger probes [92].…”

“…A further key topic was transbronchial cryobiopsy, which due to larger samples and good tissue quality without crush artefacts, is particularly helpful in histological diagnosis of interstitial lung diseases, in which forceps biopsies often fail [37, 38]. One multicentre prospective study evaluated the feasibility, safety and potential role of transbronchial cryobiopsy in the diagnosis of diffuse lung infiltrates in 17 mechanically ventilated patients [39].…”

“…Transbronchial lung cryobiopsy (TBLC) is a new diagnostic technique [92,[122][123][124][125][126]. Its diagnostic effectiveness and complication rates are varied and depend on the experience of the person performing the procedure [123,125,[127][128][129][130][131][132][133][134][135][136][137][138][139]. The mean mortality due to this diagnostic procedure only has been estimated at 0.2%, with less frequent sudden exacerbations than in the case of SLB (1.2% vs 6.1%).…”
