Transarterial Therapies for the Treatment of Intrahepatic Cholangiocarcinoma

Abstract: Cholangiocarcinoma, whether arising from the intrahepatic or extrahepatic biliary system, is a rare but devastating malignancy. Prognosis is poor, with 5-year overall survival <5% including patients undergoing surgery. Resection is the only curative treatment; however, only ?30% of patients present at a resectable stage, and intrahepatic recurrence is common even after complete resection. This article discusses the current role of transarterial therapies in the treatment of intrahepatic cholangiocarcinoma.

“…CCA is often asymptomatic in early stages, and consequently, most patients are diagnosed with advanced unresectable disease burden. The 5-year survival of unresectable CCA is 5% [5]. Risk factors include HBV and HCV infections, hepatolithiasis, nitrosamine compounds, history of primary sclerosing cholangitis and inflammatory bowel disease, and geographic risk factors such as liver fluke infections in southeastern Asia [1].…”

“…Dokudaki ortalama menzili 2,5 mm olup maksimum menzil 12 mm'dir. En büyük avantajı, eksternal radyasyon uygulamalarına kıyasla çok daha yüksek lokal dozlara ulaşan, örneğin 120 Gy'lik hedef dozlara ulaşılabilmesidir (16).…”

“…Currently, different local therapies are available for inoperable ICC. The role of trans-arterial chemo-embolization (TACE) has been investigated ( 9 ). A meta-analysis on TACE in ICC treatment has been recently published, demonstrating a survival benefit compared to standard systemic therapies, with a good toxicity profile ( 10 ) Similarly, radiofrequency ablation resulted an effective treatment option in unresectable ICC.…”
