Transanal minimally invasive surgery for total mesorectal excision (TAMIS–TME): a stepwise description of the surgical technique with video demonstration

Abstract: Transanal minimally invasive surgery (TAMIS), which can provide high-quality local excision for rectal neoplasia, has emerged as an important alternative to transanal endoscopic microsurgery. The role of TAMIS beyond local excision has also been explored, and it has been shown that this platform can be used for complete rectal resection with total mesorectal excision (TME). The stepwise approach to TAMIS for TME is described and illustrated in video format.

“…The paper by Atallah et al [1] and its video mirror the work of Lacy et al [2] who recently presented in Barcelona and in Vienna the results of the first 40 cases of what he called abdomino-transanal TME. Last year, in this very journal, Zheng published the first case of transanal TME carried out entirely from below [3] shortly followed by Leroy et al [4] who called it ''no scar transanal TME'', both using many of the same principles.…”

“…Relatively small case series describe the safety and feasibility of TAMIS for resection of adenomas and early rectal cancers, 42 but the relative infancy of the technique precludes long-term oncologic results at present. TAMIS procedures in the upper rectum 43 and in combination with transabdominal TME for low rectal cancers 44 have been described.…”

“…Several potential advantages have been postulated. Atallah was the first to describe this combination of techniques including a detailed video and the results of the 20 first cases [8,9] . Rectal distention with CO 2 combined with magnified optics permits excellent visualization of tissue planes.…”
