Transanal endoscopic microsurgery (TEM) for T1 rectal cancer

Abstract: Transanal endoscopic microsurgery (TEM) was introduced in 1983 as an alternative to radical proctectomy for the management of rectal adenomas unsuitable for endoscopic or standard transanal resection. Since, its indications have progressively broadened to early rectal cancer. This review article will assess the role of TEM for T1 rectal cancer management, along with its operative and long-term results.

“…All TES procedures were performed as previously published using the TEO ® platform (Karl Storz, Tuttlingen, Germany). In the case of suspicion of intra‐operative peritoneal perforation, a laparoscopic abdominal exploration was performed, and if a rectal perforation was found the peritoneal defect was sutured with interrupted sutures (Prolene 2/0; Ethicon, Cincinnati, OH, USA).…”

“…More than 30 years ago, TEM revolutionized the technique and outcomes of transanal surgery, its indication rapidly extending from the treatment of large rectal adenomas to early rectal cancer. More recently, its potential role in combination with neoadjuvant therapies for the treatment of more invasive cancer has been discussed.…”

“…LE was routinely performed 9-11 weeks following the end of RCT. LE consisted of a complete, en bloc and full-thickness technique, down to the peri-rectal fat, with 10 mm margins, performed by classic transanal approach for very low lesions or by TEM, as previously described [21].…”
