Transabdominal color Doppler ultrasonography for the diagnosis of small intestinal and vesical varices in a patient successfully treated with percutaneous transhepatic obliteration

Abstract: A 52-year-old asymptomatic female was admitted to our hospital in January 2008 with enlarged esophageal varices due to alcoholic liver cirrhosis. Color Doppler ultrasonographic examination showed images of vessels on the small intestinal wall around the umbilical area and the anterior wall of the bladder. Images of the vessels were obtained for the small intestinal wall and the anterior wall of the bladder with computed tomography. Percutaneous transhepatic portography was performed, and hepatofugal blood flow… Show more

“…Jejunoileal varices develop mainly at sites of prior surgeries or adhesions after surgery, mostly between the jejunum and the abdominal wall. The afferents of jejunoileal varices are mainly the tributaries of the superior mesenteric vein and the efferents are veins of the abdominal wall and also through the veins of Retzius [5658]. …”

“…Sato et al demonstrated that intramural rectal varices, perirectal collateral veins, and the communicating veins between intramural rectal varices and perirectal collateral veins could be observed clearly via an ultrasonic microprobe. 15,[59][60][61][62] The right colon is drained by three tributaries of SMV, which include the ileocolic, right colic and middle colic veins. Tributaries joining inferior mesenteric vein drain the rest of the colon.…”

“…Previously reported cases of vesical varices had a history of abdominal surgery [73, 74, 76, 77], so that the vesical varices might have appeared after surgery that provided an unusual collateral route resulting from portal hypertension. …”

“…We used PTP to reveal the detailed hemodynamics of the collateral circulation in vesical variceal patients, including the afferent and efferent veins, and these patients were successfully treated with PTO [77]. …”
