Trajectories of Heart Activity Across Infancy to Early Childhood Differentially Predict Autism and Anxiety Symptoms in Fragile X Syndrome

Abstract: Background: Fragile X syndrome (FXS) is a monogenic disorder characterized by high rates of autism spectrum disorder (ASD) and anxiety. A longstanding “hyperarousal hypothesis” in FXS has argued that ANS dysfunction underpins many symptoms of FXS. However, the developmental onset and trajectory of ANS dysfunction, as well as the consequences of ANS dysfunction on later psychiatric symptoms, remain poorly understood in FXS. Insight into the emergence, trajectory, and consequences of ANS dysfunction across early… Show more

“…The PAS-R is a 38-item, parent-report questionnaire designed as a screening and diagnostic instrument for anxiety disorders in young children. In line with other published studies (Ezell et al, 2019;Hogan et al, 2021) and to better capture the variability seen in our sample, raw scores for the social anxiety scale were utilized in the present study, with higher scores indicating more anxiety symptoms. This scale has high-internal consistency (α = 0.89) and construct validity when predicting Social Phobia diagnoses when using the Anxiety Disorders Interview Schedule (Edwards et al, 2010).…”

“…In this study, we employed temperamental measures of negative affect that reflect biologically based indices of reactivity to environmental and social challenges complemented with physiological indices of RSA that, together, capture biological competence to manage challenges ( 23 , 32 ). Negative affect and RSA have been linked to autism symptoms and social anxiety in both clinical and NT populations ( 25 , 27 , 36 , 41 , 47 ); however, the integration of these relationships has not been examined in young children with nsASD or contrasted to FXS. The present study addressed this gap in the field, and our findings indicate important patterns that are unique across clinical groups.…”

“…Interestingly, despite the connection between early negative affect and autism symptoms in children with nsASD, no such relation seems to exist in children with FXS ( 27 , 28 ). However, there is evidence to suggest that RSA in FXS increasingly diverges from NT RSA across early development ( 41 ), a trajectory that mirrors the onset of autism symptoms ( 42 ). Further, RSA at 24 months in NT males and those with FXS was found to predict autism features at 36 months, such that lower RSA was associated with more autism symptoms ( 41 ).…”
