Objectives: Methamphetamine (METH) use has been associated with impairments in a variety of cognitive functions. In this study, it was aimed to assess the relation between cognitive measures and frequency of METH use. Methods: Ninety-eight participants with methamphetamine use disorder were assessed with Wisconsin Card Sorting Test (WCST), Victoria-Stroop Word Color Test (SWCT), Trail Making Test (TMT) A and B. Participants were followed up during 6-month period with multiple urine toxicology tests (at baseline, month 1, month 2, month 3 and month 6). Results: Participants who had their prescriptions regularly were more likely to have higher rates of METH-negative urine results ( P = .003). Participants who had more correct numbers, completed more categories and had more conceptual level responses on WCST were more likely to use lower frequency of METH (OR = 0.006, P < .01; OR = 0.171, P < .001; OR = 0.024, P < .001; respectively). The higher error numbers and perseverative error rates on WCST were associated with more frequent METH use (OR = 0.023, P < .001; OR = 0.076, P < .001). Interference factor on SWCT was related with lower frequency of METH use whereas color naming factor on SWCT was related with higher rates of urine results (OR = 0.012, P < .001; OR = 3.628, P < .001; respectively). Higher TMT B-A score was related with more frequent METH use, although the significance disappeared after adjustment (OR = 0.002, P < .001). Having psychotic symptoms predicted less frequent use, but after adjustment for other significant variables, there was no significance. Conclusion: Lower frequency of METH use in the follow-up could be predicted by neurocognitive assessments. Deficits in executive functions, attention, set-shifting and mental flexibility seems to be the most effected domains and this effect may be independent from the severity of psychotic symptoms.

Introducción: El Trail Making Test (TMT) es el test más utilizado en Latinoamérica, para medir la atención sostenida (Parte A) y la función ejecutiva (Parte B). Sin embargo, los baremos utilizados muestran diferencias en las puntuaciones promedio entre grupos de poblaciones. Objetivo: Determinar los promedios referenciales del test TMT, considerando el efecto del factor sociodemográfico, a fin de proporcionar datos normativos en población ecuatoriana adulta s ana, de 18 a 90 años de edad. Método: Se utilizó un método cuantitativo, de diseño no experimental, basado en técnicas psicométricas en una muestra no probabilística de un total de 1679 participantes ecuatorianos. Se administró el test TMT, parte A y B. Resultados: El 57,4 % de la muestra correspondió a participantes femeninos, mientras que un 42,6% estuvo conformado por participantes masculinos. La mayoría provenientes de la ciudad de Quito (41,7%). La media de edad fue 34 años, y la mayoría reportaron escolaridad superior. Las puntuaciones de los promedios referenciales en la parte A para la muestra general fue (M: 34,1; DE: 13,2), en cambio en la parte B fue (M:77,9; DE: 39,8). Comparando las puntuaciones por género y por grupos de edad, no se observaron diferencias significativas; no obstante, en la parte TMT-B, el grupo con escolaridad superior (> a 12 años), obtuvo un mejor rendimiento, comparado con los grupos con menos años de escolaridad (P=0,000). Conclusiones: los promedios referenciales para el test TMT en población ecuatoriana, muestran dependencia con el factor sociodemográfico asociado a la escolaridad. Además, las puntuaciones obtenidas, se observan ligeramente por debajo de los baremos proporcionados en otras poblaciones latinas. Por tanto, sugiere la construcción de un baremo referencial para el test TMT, adaptado al contexto cultural ecuatoriano.

Introduction Alcohol dependence is characterized by a gradual reduction in cognitive control and inflexibility to contingency changes. The neuroadaptations underlying this aberrant behavior are poorly understood. Using an animal model of alcohol use disorders (AUD) and complementing diffusion-weighted (dw)-MRI with quantitative immunohistochemistry and electrophysiological recordings, we provide causal evidence that chronic intermittent alcohol exposure affects the microstructural integrity of the fimbria/fornix, decreasing myelin basic protein content, and reducing the effective communication from the hippocampus (HC) to the prefrontal cortex (PFC). Using a simple quantitative neural network model, we show how disturbed HC-PFC communication may impede the extinction of maladaptive memories, decreasing flexibility. Finally, combining dw-MRI and psychometric data in AUD patients, we discovered an association between the magnitude of microstructural alteration in the fimbria/fornix and the reduction in cognitive flexibility. Overall, these findings highlight the vulnerability of the fimbria/fornix microstructure in AUD and its potential contribution to alcohol pathophysiology. Summary Fimbria vulnerability to alcohol underlies hippocampal-prefrontal cortex dysfunction and correlates with cognitive impairment.
