Tract Embolization With Gelatin Sponge Slurry for Prevention of Pneumothorax After Percutaneous Computed Tomography-Guided Lung Biopsy

Abstract: Embolization of the needle tract during percutaneous lung biopsy with gelfoam slurry significantly decreased the odds of requiring a chest tube for PTX and should be considered for all patients, particularly those with emphysema and deep lesions.

“…Though pancreatic cysts and ovarian cysts lie in different anatomical compartments, it is encouraging that aspiration of pancreatic cysts is not associated with an increased risk of disease recurrence or mortality in patients with pancreatic cancer ( Beane et al, 2011 ; Ngamruengphong et al, 2013 ; Kudo et al, 2014 ; Ngamruengphong et al, 2015 ). Recent advancements in methods of plugging biopsy tracts to prevent tumor cell dissemination, as well as the increased use of intraperitoneal chemotherapy, further lessen the concern of tumor cell dissemination following ovarian cyst aspiration ( Armstrong et al, 2006 ; Tran et al, 2014 ; Tsang et al, 2014 ; Tewari et al, 2015 ). On the basis of these observations and recent developments, we believe that ultrasound-guided aspiration of ovarian cyst fluids would likely be a safe and well-tolerated procedure.…”

“…Prevention : Patients should remain quiet and avoid talking or coughing an appropriate needle track should be chosen, and the number of punctures should be reduced. It is not currently known whether the use of exogenously injected biogel, injectable gelatin sponge, or sterile saline to seal the needle track can effectively reduce the development of pneumothorax …”

“…Puncture site plugging may be performed when using the coaxial technique in procedures with higher risk of bleeding or to reduce the rate of pneumothorax in lung biopsies. 30,37 In this technique, the embolic material (typically Gelfoam slurry or autologous blood clot) is injected through the hollow guide needle as it is retracted to embolize the biopsy site and needle tract. A proprietary device with a hydrogel plug exists for lung biopsies and has been shown to decrease pneumothorax and chest tube insertion rates.…”
