Background: Due to different social and physical environments across Africa, understanding how these environments differ in interacting with placental disorders will play an important role in developing effective interventions. Methods: A scoping review was conducted, to identify current knowledge on interactions between the physical and social environment and the incidence of placental disease in Africa. Results: Heavy metals were said to be harmful when environmental concentrations are beyond critical limits. Education level, maternal age, attendance of antenatal care and parity were the most investigated social determinants. Conclusions: More evidence is needed to determine the relationships between the environment and placental function in Africa. The results show that understanding the nature of the relationship between social determinants of health (SDH) and placental health outcomes plays a pivotal role in understanding the risk in the heterogenous communities in Africa.

Background Neonatal mortality reduction is a global goal but its factors in high-burden nations vary and are poorly understood. This study was conducted to identify the factors affecting peri-neonatal mortality in Sao Tome & Principe (STP), one of the smallest African countries. Methods Institution-based prospective cohort study conducted at Hospital Dr. Ayres Menezes. Maternal-neonate dyads enrolled were followed up after the 28 th day of life (n=194) for identification of neonatal death-outcome (n=22) and alive-outcome groups (n=172). Data were collected from pregnancy cards, hospital records and face-to-face interviews. After the 28 th day of birth, a phone call was made to evaluate the newborn's health status. A logistic regression model was used to identify the relevant factors associated with mortality, with a level of significance α=0.05. Results The mean GA of the death-outcome and alive-outcome groups was 36 (SD=4.8) and 39 (SD=1.4) weeks, respectively. Death-outcome group (n=22) included sixteen stillbirths, four early and two late neonatal deaths. High-risk pregnancy score [cOR 2.91, 95%CI:1.18-7.22], meconium-stained fluid [cOR 4.38, 95%CI:1.74–10.98], prolonged rupture of membranes [cOR 4.84, 95%CI:1.47–15.93], transfer from another unit [cOR 6.08, 95%CI:1.95–18.90], and instrumental vaginal delivery [cOR 8.90, 95%CI:1.68–47.21], were factors significantly associated with mortality. The odds of experiencing death were higher for newborns with infectious risk, IUGR, resuscitation maneuvers, fetal distress at birth, birth asphyxia, and unit care admission. Female newborn [cOR 0.37, 95%CI:0.14-1.00] and birth weight of more than 2500 g [cOR 0.017, 95%CI:0.002-0.162] were found to be protective factors. In the multivariable model, meconium-stained fluid was significantly associated with death outcome. Conclusion Of the risk factors associated with peri-neonatal mortality in this study, most can be prevented with continuous monitoring of fetal heart and interventions to reduce perinatal asphyxia. Avoiding health-worker-related factors associated with delays in prompt intrapartum care is a key strategy for STP.

Background. “Induction of labor is not risk free, despite its importance for ending risky pregnancy compare to spontaneous onset of labor it has potential harms and it increases the rate of different maternal and neonatal complications.” Due to this WHO recommends IOL with only clear medical indications when the benefit more significant than potential harms. Even though there is a few study on IOL that address magnitude of birth outcome after IOL no study is found that determine the contributing factors to birth outcomes after IOL in Ethiopia especially in my study area, therefore the factors that affect the birth outcome after IOL need to be clearly understood. Objective: To assess the magnitude birth outcome after induced labor and associated factor among child bearing mother who deliver in NEMMCSH in the last two years (January 01, 2019 to December 31, 2020 GC). Data was collected from June 25 to July 09, 2021 GC. Methods: Hospital based retrospective cross-sectional study was conducted on 778 study participants selected by systematic random sampling technique among all child bearing mothers delivered by induction in NEMMCSH from January 01, 2019 to December 31, 2020 GC. Data was collected from patient cards, delivery registration log books and operation note books. Then data were entered and coded using EPI data version 3.1 and analyzed using SPSS version 25. Bivariate and multivariate logistic regression analysis was carried out to determine the association different potential factors with the birth outcome after IOL. Independent predictors were determined using adjusted odd ratio with 95% CL at p value < 0.05 in multivariate logistic regression analysis. Results: In this study the magnitude of still birth after IOL was 9.6%. Rural residence [AOR=3.59; 95%CI:(1.32, 9.80)], maternal chronic medical diseases [AOR=3.58; 95% CL: (1.23, 10.41)], history of previous still birth [AOR=7.45; 95%CI: (2.45, 22.38)], Partograph use [AOR=0.034; 95%CI: 0.01,0.09)], delivering < 8 hours[AOR=0.13; 95%CI: (0.03,0.56)] and delivering within 8-16[AOR=0.28; 95%CI:(0.10, 0.76)] hours were significant predictors for still birth. Conclusions: The magnitude of still birth after IOL was relatively high in the study area. Variables which increase the likelihood of still birth were, living in rural area and previous history of still birth. The recommendations also forwarded for health care provider, NEMMCSH, different stakeholders and for researchers.

Abstract: The study showed an unacceptably high rate of stillbirth in rural Nigeria. Early recognition of complications and prompt referral may reduce stillbirth rate.
