Current approaches to patient education provide generic standardized materials to all patients regardless of their demographics such as age and cognitive abilities. Thus, the effectiveness of this approach may suffer from a patient's motivation to fully engage with the material. To alleviate these concerns, this study proposes a personalized approach to patient education that is tailored to the individual characteristics and health objectives of the patient. Personalized features will enhance the comprehensibility and usability of the process of medical education. Taking this into consideration, this paper introduces a conceptual architecture to create a web-based personalized patient education experience. A key component of this architecture comprises ontological models of the patient themselves, their medical conditions, physical activities and their educational attainments. Furthermore, rule-based reasoning is also proposed to achieve this personalization. The paper proposes a software architecture for applications which use the reasoning upon SWRL enabled OWL ontologies and SQL like retrievals as core computational models. The application assists patients affected by diabetes 2, to personalize the reversibility of the condition through the diet and life style changes. The novelty is in (a) the deployment of SWRL enabled OWL ontologies in the management of data related to the personalisation of reversibility of diabetes 2 and in (b) the proposed software architecture, which contains and manipulates the SWRL enabled OWL ontology and SQL databases at the same time and transparently. The application, which has been implemented within the Java environment and NetBeans, is reusable in any other problem domain when the personalization of healthcare delivery is required. The proposed architecture also generates applications within Android environments without changing its style and the computational models.

Diabetes is an endocrine chronic disease that causes high blood sugar level, produced by retardation in, or deficiency of, glucose metabolism in the body of the individual with the disease. Neuropathies and/or angiopathies are complications of diabetes that result in changes in the lower limbs which subsequently evolve for the diabetic foot. Diabetic foot represents one of the most devastating complications of diabetes and can lead to ulcerations, amputations and even death. Based on these, the aim of this work was to develop an Intelligent System for Monitoring the Prevention of Diabetic Foot (SIM2PeD), allowing personalized care from each individual routine. The work consists of a platform integrated with a mobile device to capture individuals' data, entitled Mobile SIM2PeD, and a web device for monitoring the medical patient, titled Web SIM2PeD. Individuals receive alerts regarding care according to their location and activity directly from their smartphones. After capturing, the information is passed to the expert system (Intelligent module) that generates recommendations from the answers. The developed system presents a model of alerts as the best architecture, to the detriment of the pictogram model. The data captured show that slight displacements in frequency caused large variations of answers delivered to the application. The various experiments conducted made the system performed to be specified, and suitable for the remote monitoring of self-care activities in patients with diabetic foot.
