Towards a medical school curriculum for uncertainty in clinical practice

Abstract: Uncertainty abounds in the clinical environment. Medical students, however, are not explicitly prepared for situations of uncertainty in clinical practice, which can cause anxiety and impact well-being. To address this gap, we sought to capture how students felt in various clinical scenarios and identify programs they found helpful as they worked through uncertainty in their clerkships to better inform curriculum that prepares them to acknowledge and navigate this uncertainty. This is an observational cross-se… Show more

“…Thus, healthcare educators could begin integrating uncertainty pedagogy across a curriculum through purposefully planned experiential learning approaches (e.g., case questions and simulations). This suggestion is further supported by recent findings exploring which types of pre-clinical learning which was perceived as enhancing uncertainty tolerance (Papanagnou et al, 2021). Herein, small group learning and simulations were identified as teaching practices fostering uncertainty tolerance, with simulations reported as helping students to "realize real life is much more fluid and less concrete.…”

“…After medical school, a physician also often lends himself to a chronically stressful lifestyle ( 16 ). Owing to the COVID-19 pandemic, medical students are unprepared for situations of uncertainty in clinical practice, which can generate anxiety and stress and impact mental wellbeing ( 17 ). Therefore, simple questionnaires are needed to enable appropriate screening of these problems, as timely detection of anxiety in medical students is critical to proposing specific interventions to maintain their mental health and ensure proper development of their academic activities and future professional work.…”

“…However, medical students are not fully prepared for uncertainty in clinical practice, a situation that can lead to anxiety and compromise health. Strategically integrating speci c forms of education such as storytelling, role play, and presentation into the formal curriculum may help develop the skills students needed to cope with uncertainty [27]. The mCBL-based teaching model relies on teaching cases of selected diseases with the help of microlearning, e-books, and other information platforms to promote active learning and improve students' clinical integration skills.…”
