Towards a dynamic customised therapy for ocular surface dysfunctions

Abstract: The ocular surface is a highly dynamic system characterised by continuous adaptive reactions to environmental, toxic, infective, traumatic and inflammatory conditions that interfere with its homeostasis. When the adaptive response is not able to cope with environmental or internal aggressions, a persisting dysfunction may develop into a self-maintaining vicious cycle, thereby developing a chronic disease. This process is not even, but characterised by cycles of worsening and improvement accompanied by morphofu… Show more

“…However, reliable assessment of DED severity can be problematic due to several issues, including poorly standardised evaluation methods, non-correlation between disease severity and clinical/biological disease markers, and individual variability in symptomatology and disease signs. The vicious circle of DED pathogenesis, which can exacerbate the condition and facilitate merging or development of mechanistically distinct DED subtypes can further hinder accurate evaluation 21 13 22 42…”

“…6,7 If the inflammatory response is self-limiting, it can hasten restoration of corneal epithelial integrity and function by promoting wound healing through suppressing pathogenic infiltration. 8 Even though some of the sequelae subsequent to hypertonicity-induced ROS generation have been characterized, it is unknown whether transient receptor potential melastatin 2 (TRPM2) channels are a biosensor of hyperosmotic and oxidative stress and trigger activation of this signaling axis in human corneal epithelial cells (HCECs).…”

“…164 The need for patientoriented therapy has also been claimed for postmenopausal HT 165 and ocular surface diseases. 166,167 Hormonal supplementation stands upon the basic consideration that a lowering in bioavailable hormone(s) exists in a given system. Clinical characteristics and potential biomarkers should be identified to guide the HT strategy in those DE suffering women who would be most likely to benefit from treatment.…”
