Toward the Reliable Diagnosis of Indeterminate Thyroid Lesions: A HRMAS NMR-Based Metabolomics Case of Study

Abstract: Cytological analysis of thyroid nodules detected using ultrasound-guided fine-needle aspiration technique is an efficient method for the diagnosis of well-differenciated tumors such as papillary thyroid carcinoma. However, for between 10 to 30% of all the nodules, the cytological analysis based on fine-needle aspiration biopsies leads to an "indeterminated" identification. Consequently, a surgical excision is then necessary for a definite histological diagnosis of the lesions, resulting in 85% of the patient w… Show more

“…These results were consistent with a previous report by Torregrossa et al [21]. Lower concentrations of fatty acids could be caused by an increase in metabolic rate and enhanced membrane synthesis in the cancer tissue [21].…”

“…Notably, Caldarelli's team used an HRMAS-NMR method to build a diagnostic model to discriminate malignant tumors from the benign ones [21, 22]. The resulting model has better sensitivity and specificity compared to the gold-standard FNA method.…”

“…The authors suspected that a group of follicular lesions might progress through early malignant changes as was previously reported for adenoma-carcinoma in colonic neoplasms [27,28]. Most recently, the potential of HR MAS spectroscopy measurements to achieve highly accurate diagnosis in thyroid tumor fine-needle aspiration biopsies has been reported in the literature [29,30]. This approach enables discrimination not only between normal and malignant tissues but also between malignant and benign lesions.…”

“…The feasibility of using proton MR to study not only thyroid tissue but also FNA was investigated few years ago [103]. Later, a metabolomic approach based on nuclear magnetic resonance showed its potentiality to provide data from cytological thyroid specimens [104]. These authors highlighted an increase of phenylalanine, taurine, and lactate levels and a decrease of choline and choline derivatives, myo-and scylloinositol in the malignant tumors compared with the benign tissue.…”
