Total Stapled, Total Intra-Abdominal (TSTI) Laparoscopic Roux-en-Y Gastric Bypass: One Leak in 1,000 Cases

“…BAND-to-RYGBP revision patients is comparable to that reported in primary (initial) RYGBP series [3,17,23] and is lower than that reported in revision series from vertical-banded gastroplasty (VBG) or silastic ring vertical gastroplasty (SRVG) to RYGBP [4,7,8,11,19,20,24]. It is especially important to note that no leaks or other serious complications occurred in our series of revision patients.…”

“…Endoscopic surgical stapling devices (henceforth, staplers) were introduced to facilitate tissue approximation and transection during endoscopic surgery 12 . Staplers simplify the procedure and require less skill of the surgeon, but have been associated with complications, including leakage from staple lines, bleeding, and fistula formation [13][14][15] . The first powered stapler (i.e.…”

“…Although these devices are generally safe and effective, the correct closed staple limb length must be selected for the thickness of the tissues involved and the device must provide the correct tissue compression [5]. Staple-line failures in gastric bypass operations have been reported in 0.1-5.3% of cases [9][10][11] and may lead to peritonitis and/or abscess formation. If not managed successfully, staple-line failure could cause severe sepsis, multiple organ failure, and even death [10].…”
