Total Lower Lip Reconstruction With Functional Gracilis Free Muscle Flap

Abstract: Free flaps based on static slings principles cannot provide esthetic and functional outcomes at a desired level in total or close to total lip loss. Therefore, dynamic methods have become a current issue in recent years and especially the idea of functional gracilis free muscle flap has been suggested. In this study, we present a case of a successful total lower lip repair with this flap.In a 78-year-old female patient who was diagnosed with squamous cell carcinoma involving the entire lower lip, bilateral mod… Show more

“…The lips play an important dynamic for aesthetics and functional aspects, as well as communication, sound production, facial expressions, swallowing and preservation of lip sealing 1,2 . Extensive lip injures are a challenge for surgical reconstruction due to the need to reestablish the lip competence and many approaches or procedures can be performed 3 . And even minor lip defects require meticulous reconstruction to minimize injury defects because the lips are within the field of observation of the face 4 .…”

“…10 The last report of this innervated free flap was in 2018 by Cakmak et al for a squamous cell carcinoma of the lower lip using a skin graft for the outside layer. 11 As a first step, we attempted a replantation. Replantation of avulsed lower lip was successfully reported in 1988 by Schubert et al, 12 but, in our case, probably the intimal damage of the artery caused by the tear of the bite led to microthrombi formation.…”

“…Hand trauma is a frequent injury in life because humans need their hands for productive activities and life all the time. 1 , 2 Patients suffering from bone and joint injuries, such as severe hand open skin injuries, tendon injuries, nerve and blood vessel injuries, must undergo surgery in time, and such patients are called patients with severe hand trauma. 3 When a patient suffers from hand trauma, different fingers or even multiple fingers are often involved.…”
