Total laparoscopic subtotal gastrectomy with transvaginal specimen extraction is feasible in advanced gastric cancer

Abstract: HighlightsLaparoscopic gastrectomy had satisfactory results in patients with gastric cancer.Natural orifice surgery (NOS) is an ever-evolving advanced laparoscopic technique.NOS promises minimizing surgical injury, less wound complications and faster recovery.Laparoscopy plus NOS can potentiate the advantages of both minimal invasive techniques. We presented a transvaginal extraction of an advanced gastric cancer after laparoscopic gastrectomy.

“…[7][8][9] Besides this present case colon specimen from right hemicolectomy, gastric specimen from subtotal gastrectomy, liver specimen from liver resection and partial pancreas specimen from distal pancreatectomy are reported to be obtained from vaginal wall incision during NOSE. [10][11][12][13] Luyao Zhang et al have successfully performed laparoscopic pancreaticoduodenectomy through transvaginal NOSE method. [14] Although advantages of laparoscopic transvaginal NOSE are well recognize, some risks and complications have been observed and drawn attention by surgeons recently.…”

“… 3 It was previously demonstrated that laparoscopic gastrectomy combined with NOSE for advanced gastric cancer may provide a smooth postoperative course without any wound-related complications. 4 Additionally, we systematically reviewed 90 cases of laparoscopic right colon resections with transvaginal extractions and found no woundrelated complications. 5 Our preliminary observations pointed out that natural orifice surgery may decrease the risk of abdominal wound-related complications.…”

“…Moreover, the risk of thromboembolism may also be reduced if the recovery period of the patients can be further shortened with smaller wounds and fewer wound complications. 4 5 Lastly, decreasing the postoperative woundrelated complications can offer earlier initiation of the postoperative chemo-radiotherapy, or, at least, it may avoid any delay for adjuvant therapies due to wound-related complications. 4 5 …”

“…Her iki çalışmanın eksik yönü ise yöntemlerini geleneksel yöntemlerle kııyaslamamış olmalarıydı. Laparoskopik bazı çalışmalarda ise transvajinal olarak veya uç dikiş tekniği ile piyeslerin çıkarılması farklı modaliteler olarak sunulmuş fakat cerrahlar ve hastalar tarafından rağbet görmemiştir [19][20][21] .…”
