Total Hip Arthroplasty in the Very Young Patient

Abstract: The surgical management of end-stage hip disease in patients aged <30 years remains a challenge. Hip-preserving surgical procedures in the setting of advanced disease often do not provide adequate pain relief, but the implications of joint arthroplasty surgery in the very young patient are a matter of concern. The outcome of total hip arthroplasty (THA) in these patients varies, largely because of the wide spectrum of diagnoses associated with hip disease in this group, the complexity of deformities requiring … Show more

“…Revision surgery to address such failure involves increased risks, complications, and costs. Given that the average life expectancy of many patients undergoing total hip arthroplasty (THA) exceeds 25 years, ongoing stress shielding is clinically important and the demand for implants which maximize the load-bearing capabilities of the bone is high [16, 17]. …”

“…Young patients present a unique dilemma for bearing survivorship in that they not only live longer, but commonly pursue more physically demanding activities that place significantly more stresses across the articulation surfaces [12, 42]. Currently, few reports in the literature focus on long-term results of HXLPLE in younger cohorts [3], which is an important issue to clarify due to recent problems with hip resurfacing, metal-on-metal, and primary modular implants.…”

“…2,3 Although hip replacement is an effective approach to relieve pain and improve hip function in the short term, its long-term performance remains unsatisfactory. 4,5 Furthermore, non-traumatic ONFH typically occurs in young adults and involves both hips. On the other hand, not all ONFH will progress to femoral head collapse.…”
