Necrobiotic xanthogranuloma (NXG) is a rare granulomatous condition that is often associated with a paraproteinemia and in some cases multiple myeloma. We report a case of isolated NXG that responded very well to total glucosides of paeony treatment. Characteristic clinical and histological features of NXG are presented, as well as a discussion regarding management and the use of glucosides of paeony. Treatment with total glucosides of paeony is an effective, safe treatment, which avoids the side effects associated with systemic corticosteroid or cytotoxic agent therapy.

“…Its expression was significantly increased in a variety of skin diseases, such as psoriasis, alopecia areata, vitiligo, atopic dermatitis, Henoch‐Schönlein purpura (HSP), pemphigus, and drug‐induced hypersensitivity syndrome/drug reaction with eosinophilia and systemic symptoms (DRESS) . Due to the similar pharmacological efficacy to corticosteroids but with milder side effects, Glycyrrhizin has been widely utilized in several dermatologic disorders for many years, such as active‐stage generalized vitiligo, psoriasis, alopecia areata, and is also reported to be effective in alleviating the symptoms of atopic dermatitis and systemic sclerosis in animal model . Based on the clinical observation that patients with recalcitrant Riehl’s melanosis may present with facial erythema and itching, which are signs of local inflammation, we applied Glycyrrhizin compound 75 mg orally three times a day for all enrolled patients, aiming to alleviate the inflammatory reaction.…”

Summary Background Up till now, there is no standardized and satisfactory treatment strategy for Riehl’s melanosis. Objective In this pilot study, we evaluated the efficacy and safety of a novel combination therapy with oral administration of tranexamic acid (TA) and Glycyrrhizin compound for recalcitrant Riehl’s melanosis. Methods Ten patients with Riehl’s melanosis were recruited in this study. After elimination of potential contraindication, all patients were treated with 500 mg TA together with 150 mg Glycyrrhizin compound per day orally for 3 months, followed by 500 mg TA per day orally alone for another 3 months. Lesions were imaged by reflectance confocal microscopy (RCM), dermatoscopy, and VISIA® Complexion Analysis System monthly. Mexameter was adopted to evaluate Melanin Index (MI) and Erythema Index (EI). Clinical outcome scores were given by both physicians and patients. Results Seven out of ten patients received “marked improvement”, while two received “moderate improvement” and one “minimal improvement” at the final visit. Both mean MI and EI were significantly decreased compared with baselines. Furthermore, RCM and dermatoscopy analyses confirmed the improvement of pigmentation and erythema with decreased pigment granules and telangiectatic vessels. Conclusion Oral administration of TA in combination with Glycyrrhizin compound may be an effective therapy for Asian patients with recalcitrant Riehl’s melanosis.

Alopecia areata is a chronic, recurrent and non-scarring alopecia. The prognoses of patients are very diverse. The larger the area of hair loss, the poorer the treatment response and greater the probability of chronic disease progression. Numerous treatments have been introduced, but curative treatments have yet to be established. The long-term efficacy of the current treatments is minimal, and the therapeutic response varies widely. Recent clinical trials have attempted to apply therapeutic metrics, such as the Severity of Alopecia Tool, and many have been designed as randomized controlled studies, enabling a more precise evaluation of existing treatments. There have been updates in practice, efficacy or indications of therapeutics that have been previously used. Moreover, the use of novel treatments such as biologics has recently been introduced. Commonly, the most important factor in determining the treatment modality for alopecia areata has been the extent of hair loss. However, if the disease activity is high and likely to progress, combination therapy with adjuvant modalities will be more desirable. This review will discuss the therapeutic effects of existing and newly-introduced treatments based on their quantity, quality of evidence and expected complications. In addition, an algorithmic approach to management of alopecia areata is proposed according to clinical subtype, severity, onset and activity of the disease.
