Total fecal diversion by the temporary skin-level loop transverse colostomy

Abstract: The temporary skin-level loop transverse colostomy has been described. Proof of its ability to divert the fecal stream totally is illustrated by complete diversion in 25 patients ingesting a postcolostomy barium meal. There was no postoperative complication.

“…Faecal diversion with colostomy is generally accepted as the standard treatment for extraperitoneal rectal injury. Rombeau et al 19 showed that a properly performed temporary skin‐level transverse loop colostomy completely diverts the faecal stream. A loop colostomy has the added advantage of rapidity of construction and ease of closure by avoiding a laparotomy.…”

“…The concept that a loop colostomy or proximal and distal stomas placed directly next to each other does not completely divert the fecal stream is simply false. This was elegantly depicted by Rombeau et al in which barium impregnated oral feeds given to patients with loop colostomies showed no evidence of barium spillage into the distal limb of the diverted colon (10).…”

“…Inicialmente propuesta a cabos divorciados, posteriormente se constato que la colostomía en asa sobre varilla, con buen espolón, podía igualmente desfuncionalizar el recto 46 . La literatura también coincide en que la ausencia del cierre primario de esa herida no aumentaría la morbilidad 7,34,36 .…”
