Total Endovascular Aortic Arch Repair: From Dream to Reality

Abstract: The gold-standard therapy for the treatment of aortic arch pathologies is conventional open surgery. Recently, total endovascular aortic arch replacement with branched stent-grafts has been introduced into clinical practice with the aim of reducing invasiveness especially in selected high-risk patients. The aim of this review is to describe the two most commonly used branched devices for endovascular arch stent-grafting: Nexus (Endospan, Herzlia, Israle) and RelayBranch (Terumo Aortic, Glasgow, United Kingdom)… Show more

“…In recent years, various surgical techniques have been formulated to simplify the c-TAR and proved to have encouraging outcomes ( 17 ). For instance, the triple-branched stent graft technique, which truly made the surgical procedure easier, time-saving and less invasive ( 18 ).…”

“…Thoracic endovascular aortic repair (TEVAR) combined with carotid-to-subclavian artery bypass is an appropriate option for certain patients [ 6 ]. For those requiring full arch replacement but who are unsuitable for open surgery, also total endovascular arch repair (TER) should be considered [ 11 ]. Finding the best strategy represents a major challenge, especially in the case of emergency interventions.…”

“…The management of aortic arch dissection, alternatively known as non-A non-B aortic dissection, poses a complex challenge for cardiac surgeons. Conventional open surgery remains the standard treatment approach for this condition, which typically involves deep hypothermic circulatory arrest with selective cerebral perfusion [1]. However, in selected high-risk patients, thoracic endovascular aortic arch replacement (TEVAR) with isolated or two-branched stent grafts has been introduced into clinical practice [2,3].…”

However, in selected high-risk patients, thoracic endovascular aortic arch replacement (TEVAR) with isolated or two-branched stent grafts has been introduced into clinical practice [2,3]. Despite this, the complete TEVAR procedure with triple-branched stent graft in acute arch dissection involving an entry tear in the aortic arch has not yet been described.…”
