Torture is widely practiced throughout the world. Recent studies indicate that 50% of all countries, including 79% of the G-20 countries, continue to practice systematic torture despite a universal ban. It is well known that torture has numerous physical, psychological, and pain-related sequelae that can inflict a devastating and enduring burden on its victims. Health care professionals, particularly those who specialize in the treatment of chronic pain, have an obligation to better understand the physical and psychological effects of torture. This review highlights the epidemiology, classification, pain sequelae, and clinical treatment guidelines of torture victims. In addition, the role of pharmacologic and psychologic interventions is explored in the context of rehabilitation.

ASS CONFLICT AND DISplacement continue to affect large numbers of people worldwide. 1 In 2007, the United Nations High Commissioner for Refugees documented 16 million refugees, 26 million internally displaced persons, and 12 million stateless persons. 2 Yet controversy about the mental health needs of conflictexposed populations persists, 3 a problem that is magnified by the wide variability in rates of the most commonly studied psychiatric conditions among affected populations, namely, posttraumatic stress disorder (PTSD) and depression. Prevalence rates for PTSD have ranged from 0% in a conflict-affected region of Iran 4 to 99% in Sierra Leone 5 and between 3% and 86% for depression across surveys. 6,7 Identifying the potential methodological and substantive factors that are associated with this variation is vital to determining with any accuracy rates of mental health problems of conflict-affected populations.Methodological factors, particularly the approach used for sampling and diagnosis, appear to exert a large influence on the prevalence rates yielded by surveys. 8 An outstanding question, how-Author Affiliations are listed at the end of this article.

RESUMO: O número de refugiados no mundo não tem parado de aumentar nos últimos anos. Os refugiados diferem dos outros migrantes por terem sido forçados a deixar os seus países e a eles estarem impedidos de regressar, gozando por essa razão de um estatuto garantido pelo Direito Internacional. A estes juntam-se ainda pessoas que enfrentam riscos similares mas para as quais ainda não foi apurado o seu estatuto. São populações que apresentam uma maior prevalência de problemas de Saúde Mental e que devem ter um tratamento especial. Fez-se uma pesquisa na literatura biomédica, das revisões sistemáticas publicada nos últimos dez anos, sobre Saúde Mental em Refugiados concluindo-se que estes apresentam particularidades especiais que devem ser tidas em conta pelos profissionais e serviços de saúde em termos de prevenção primária, secundária e terciária. Discutem-se questões como a barreira da língua, as diferenças culturais e o papel dos tradutores e mediadores culturais, a organização dos serviços, a intervenção em grupos especialmente vulneráveis como são as crianças, as mulheres, as grávidas e os idosos e abordam-se os problemas mais prevalentes resultantes da exposição ao trauma que assumem nos refugiados particularidades muito específicas. Palavras-Chave: Refugiados, Saúde Mental, Trauma, Revisão ______________________________________________________________________ REFUGEES AND MENTAL HEALTH -HOSTING, UNDERSTAND AND TREAT ABSTRACT:The number of refugees in the world has not stopped increasing in recent years. The refugees differ from other migrants because they were forced to leave their countries and they are prevented from returning, benefiting therefore, from a status guaranteed by international law. To these must be added people who face similar risks but whose status has not be yet been determined. These are populations with a higher prevalence of mental health problems and should have a special treatment. Research in biomedical literature available, systematic reviews published in the last ten years on refugee's mental health led to conclude that these have special characteristics that must be taken into account by professionals and health services in terms of primary, secondary and tertiary prevention. Issues addressed include language barriers, cultural differences and the role of translators and cultural mediators, the organization of the services, the intervention in particularly vulnerable groups such as children, women especially pregnant women and the elderly. Abstract: Torture prevention and interdisciplinary rehabilitation of survivors must receive more attention, and healthcare professionals have an obligation to take a substantial role in this effort.
