Topical wound anaesthesia in children – a temporary postoperative pain relief

Abstract: A very short and clinically insignificant pain relief is obtained following the administration of a lidocaine aerosol in the surgical wound. A low pain level in this model may limit the possibility to detect an effect of lidocaine.

“…Local anaesthetics can provide rapid and prolonged anaesthesia when applied to open wounds and can also prevent or reduce the subsequent pain escalation response . The vasoconstrictive properties of adrenaline slow the blood flow to the wound, thereby suppressing the inflammatory cascade, resulting in a reduction in associated pain caused by accumulation of inflammatory mediators, including cytokines and histamines .…”

“…Local anaesthetic agents act directly on nerve tissue to elicit a reversible block of the conduction of signals responsible for the sensation of pain. When applied to open wounds they can effect rapid and profound local anaesthesia 9,19–24 . They can also prevent or reduce the subsequent pain escalation response 25–27 .…”

“…When applied to open wounds they can effect rapid and profound local anaesthesia. 9,[19][20][21][22][23][24] They can also prevent or reduce the subsequent pain escalation response. [25][26][27] Although it may seem counterintuitive to apply local anaesthetic after a procedure, there are significant advantages to this approach in the sheep farm setting.…”

“…On the other hand, a metered-dose lidocaine pump spray is a regular component of the drugs kept on the emergency cart for airway anesthesia. This spray is reported to provide effective postoperative analgesia when applied to herniorrhaphy wounds [3]. Lidocaine pump spray may produce prompt cutaneous topical anesthesia that is readily available and may be an alternative to the lidocaine patch for skin analgesia prior to injection.…”
