Topical calcitriol restores the impairment of epidermal permeability and antimicrobial barriers induced by corticosteroids

Abstract: We found that topical calcitriol restored both the epidermal permeability and antimicrobial barrier that had been impaired by corticosteroids. This restoration was mediated by both an activation of the cutaneous vitamin D pathway and an increase of epidermal lipids and antimicrobial peptides, promoted by the formation of the LB and the activity of epidermal lipid synthesis-related enzymes.

“…Although decreased expression of AMP is related to increased susceptibility to cutaneous infections in atopic skin, excessive and uncontrolled expression of AMP may be involved in the induction and propagation of inflammation in psoriatic skin14. Thus, as dysregulated expression of AMP may be important for the pathogenesis of various chronic inflammatory diseases, controlling and balancing AMP expression may offer a novel therapeutic approach15. Human mesenchymal stem cells have direct antimicrobial activity, which is mediated in part by the secretion of human cathelicidin hCAP-18/LL-3716.…”

“…Besides these hallmarks and its cell-proliferative regulation properties [14–17], topical calcitriol is able to restore the epidermal permeability and antimicrobial barrier via activation of the cutaneous vitamin D pathway and also to increase epidermal and antimicrobial peptides [23]. These pharmacological features could explain the improvement seen in the calcitriol treated side.…”

“…In contrast, studies mostly performed in mice suggest that vitamin D receptor (VDR) agonists have opposing effects on many of the cellular and molecular mechanisms underlying glucocorticoid-induced skin atrophy, including effects on cell proliferation, epidermal lipids and antimicrobial peptides [911, 14]. Combination of betamethasone with calcipotriol has proven to be an effective topical treatment of psoriasis vulgaris that can be used for up to a year with very low incidence of skin atrophy [19].…”
