Tophaceous gout with rare involvement of the patella

Abstract: Gout arthritis is an inflammatory arthritis caused by hyperuricemia from various etiologies. It is typically a polyarticular arthritis with the most common joint involved being the first metatarsophalangeal joint. We present a case of gout of the knee with characteristic magnetic resonance imaging findings and rare involvement of the patella. Radiographic findings of gout are diagnostic; however, they manifest many years after the onset of the disease process. Control of symptoms and progression of the disease… Show more

“…В некоторых научных статьях, посвященных внутрикостным подагрическим тофусам, описывалась манифестная клиника подагры с ее лабораторным подтверждением, и отсутствовала связь образования кист с инъекциями ГКС [41,42,43,44,45,46,47,48,49]. Статей, описывающих локализацию тофусов в проксимальном отделе плечевой кости, не было обнаружено.…”

“…8 MRI will also show bony erosions (if present), bone marrow and/or soft-tissue edema, synovial pannus, and gouty tophi in chronic cases. 9 Figure 1, A and B shows corresponding radiographic and MRI of periarticular erosions and gouty tophi in the thumb. Tophi may appear hypointense or isointense on T1, variable signal on T2, and enhance homogeneously with contrast.…”

“…28 Gouty tophi have been described in the patellar tendon and inside the patella itself, with the recommended treatment being uric acid-lowering therapies to prevent pathologic tendon rupture or pathologic patellar fracture. 9 Outcomes After Total Hip Arthroplasty/TKA…”

“…The first metatarsophalangeal joint is the most commonly involved joint, although gout is often polyarticular. Involvement of the knee is not uncommon, but involvement of the patella is rare [34,35]. Isolated involvement of patella in gout is extremely unusual.…”
