Intravascular optical coherence tomography (IV-OCT) is an imaging modality that can be used for the assessment of intracoronary stents. Recent publications pointed to the fact that 3D visualizations have potential advantages compared to conventional 2D representations. However, 3D imaging still requires a time consuming manual procedure not suitable for on-line application during coronary interventions. We propose an algorithm for a rapid and fully automatic 3D visualization of IV-OCT pullbacks. IV-OCT images are first processed for the segmentation of the different structures. This also allows for automatic pullback calibration. Then, according to the segmentation results, different structures are depicted with different colors to visualize the vessel wall, the stent and the guide-wire in details. Final 3D rendering results are obtained through the use of a commercial 3D DICOM viewer. Manual analysis was used as ground-truth for the validation of the segmentation algorithms. A correlation value of 0.99 and good limits of agreement (Bland Altman statistics) were found over 250 images randomly extracted from 25 in vivo pullbacks. Moreover, 3D rendering was compared to angiography, pictures of deployed stents made available by the manufacturers and to conventional 2D imaging corroborating visualization results. Computational time for the visualization of an entire data sets resulted to be ~74 sec. The proposed method allows for the on-line use of 3D IV-OCT during percutaneous coronary interventions, potentially allowing treatments optimization.

Introduction The complexity of the aortic valve and aortic root is appreciated, especially by specialists in medical imaging and by surgeons who devise and perform aortic valve repair. However, the terminology used to describe the components of the valve differs between different specialists and even within one group of specialists. The aim of this review was to discuss the aortic valve and its root. Discussion The following structures need proper labelling before the root itself can be described unequivocally: valve leaflets, commissures, sinuses of Valsalva, interleaflet triangles, sinutubular junction and ventriculo-aortic junction. Especially the latter deserves attention since there is an anatomic as well as a haemodynamic junction. The difference between both junctions is the key to understand the aortic valve. Its understanding is also of vital importance for surgeons in performing durable aortic valve reparations. Conclusion The differences in terminology of the components of the aortic valve are probably long lasting. Therefore, a clarifying definition of every component described in any scientific manuscript should be provided.

Intravascular optical coherence tomography (IV-OCT) is a new intervention imaging technology for the diagnosis of coronary heart disease. Because of its high-resolution and non-invasive characteristics, it is widely used in clinical medicine. It has a revolutionary impact on coronary artery disease diagnosis, prognosis and treatment decisions. This paper studied this hot topic, gave the basic concepts of intravascular OCT image sequences. In accordance with current development studies, the key technology for three-dimensional reconstruction of the coronary vascular system is proposed and analyzed. On this basis, we proposed future research directions on IV-OCT vascular three-dimensional reconstruction, including clinical trial frequency domain OCT (OFDI), the combination of IV-OCT and intravascular ultrasound (IVUS) and a preliminary study of intravascular photoacoustic (IVPA) imaging.
