Red palm super olein (RPSO) merupakan fraksi cair yang dihasilkan melalui fraksinasi berulang dari red palm oil (RPO). RPSO mengandung senyawa fitonutrien (karoten, vitamin E, skualena) yang tinggi. Beberapa tahun terakhir, tokotrienol mendapat perhatian yang tinggi karena sifat biologisnya yang multitarget, termasuk sebagai perlindungan saraf, antiinflamasi, antioksidan, dan antikanker. Tokotrienol menunjukkan aktivitas antiinflamasi yang signifikan dalam berbagai penyakit pada manusia, di antaranya penyakit kanker, kardiovaskular, neurodegeneratif, dan reumatik. Selain itu, tokotrienol dilaporkan sebagai agen antiinflamasi yang lebih kuat dibandingkan dengan tokoferol. Hal ini disebabkan karena tokotrienol memiliki rantai samping yang tidak jenuh, serta dapat menurunkan nilai C-reactive protein (CRP) 20—50% lebih banyak dari tokoferol yang mempunyai rantai samping yang jenuh. Namun, terdapat beberapa tantangan dalam pemanfaatan tokotrienol sebagai antiinflamasi, yaitu bioavailabilitas oral yang rendah, seperti cepat mengalami metabolisme dan ekskresi. Tulisan ini mengkaji potensi tokotrienol pada RPSO sebagai antiinflamasi pada reumatik, aktivitas antiinflamasi tokotrienol pada penanganan reumatik, tantangan pemanfaatan tokotrienol dalam pengobatan, dan pengembangan bentuk sediaan berbasis tokotrienol yang memberikan efek terapi optimal.

Background: Honey has been used for centuries for its medicinal benefits, where topical treatments in the folklore literature reported its beneficial role in treating different types of wounds. Despite its immense use in wound care, honey’s exact mechanism remains unclear. Purpose: The present study aimed to explore and abridge the gap between honey and its molecular wound healing mechanism. Methods: An extensive literature study has been performed where these activities can be explained due to phenolic compounds, nitric oxide, non-peroxide factors, low pH, high osmolarity, and hydrogen peroxide in honey. Besides that, honey also contains carbohydrates, proteins, amino acids, lipids, minerals, and vitamins, contributing to the wound healing potential to some extent. However, these metabolites and constituents differ among different honey varieties due to the distinct nutritional requirements of different species of bees and their collection of nectar from varied sources containing different natural actives. Results: The revival of honey-based wound care research has identified a few probable mechanistic pathways of honey primarily due to its anti-inflammatory, antioxidant, antimicrobial, and immunomodulatory potentials. It also promotes angiogenesis, facilitates reepithelialization, stimulates proliferation of extracellular matrix, reduces neutrophil formation, modulates production of TNF-α, IL-1β, and IL-6, and prevents secondary infection at the wound site. Conclusion: The varieties of honey vary in their qualities, phenolic compounds, and safety for human use. A few of these are currently clinically employed for wound dressings. Moreover, honey can be effective for managing complicated and chronic wounds; more extensive molecular research and safety profiling would be necessary.

Hypertrophic scarring is an aberrant wound-healing response to reestablish dermal integrity after an injury and can cause significant abnormalities in physical, aesthetic, functional, and psychological symptoms, impacting the patient's quality of life. There is currently no gold standard for preventing and treating hypertrophic scars. Therefore, many researchers have attempted to search for antihypertrophic scar agents with greater efficacy and fewer side effects. Natural therapeutics are becoming attractive as potential alternative anti-scarring agents because of their high efficacy, safety, biocompatibility, low cost, and easy accessibility. This review demonstrates various kinds of natural product-based therapeutics, including onion, vitamin E, Gotu kola, green tea, resveratrol, emodin, curcumin, and others, in terms of their mechanisms of action, evidence of efficacy and safety, advantages, and disadvantages when used as anti-scarring agents. We reviewed the literature based on data from in vitro, in vivo, and clinical trials. A total of 23 clinical trials were identified in this review; most clinical trials were ranked as having uncertain results (level of evidence 2b; n = 16). Although these natural products showed beneficial effects in both in vitro and in vivo studies of potential anti-scarring agents, there was limited clinical evidence to support their efficacy due to the limited quality of the studies, with individual flaws including small sample sizes, poor randomization, and blinding, and short follow-up durations. More robust and well-designed clinical trials with large-scale and prolonged follow-up durations are required to clarify the benefits and risks of these agents.
