Tocilizumab Is Associated with Increased Risk of Fungal Infections among Critically Ill Patients with COVID-19 and Acute Renal Failure: An Observational Cohort Study

Abstract: Research Question: Does treatment with tocilizumab increase the risk of a fungal infection in critically ill patients with coronavirus-19? Background: Numerous therapies have been evaluated as possible treatments for coronavirus-2019 caused by severe acute respiratory syndrome coronavirus-2. Tocilizumab is a humanized monoclonal antibody directed against the interleukin-6 receptor that has found a role as a therapy for patients with severe coronavirus-19 pneumonia. The immunomodulatory effects of tocilizumab m… Show more

“…Up to date, there is no sufficient and effective treatment option to prevent and protect against COVID-19-asociated AKI in patients ( Jansen et al, 2022 ). Drugs such as corticosteroids, tocilizumab and remdesivir exhibit no significant benefit for kidney or mortality in clinical studies ( Kumar et al, 2022 ; Burger et al, 2023 ; Grundeis et al, 2023 ). Kidney replacement therapy (KRT) is currently the only treatment for severe AKI.…”

“…Also, a single patient recently received dupilumab for the treatment of severe asthma and atopic dermatitis and an incident of Candida infection occurred. A total of 303 patients received IL-6 inhibitors and 12 had Candida infection including five candidemia with tocilizumab (400 mg) after two weeks ( 51 , 52 , 97 , 98 ).…”

“…In another retrospective cohort study, tocilizumab (400 mg) was given to 65 patients for the treatment of severe COVID-19, and one patient had candidemia day 18 ( 97 ). In another example of an observational cohort study, 16 patient with COVID-19 were administered with tocilizumab (8 mg/dose) and five patient had candidemia ( 98 ). Furthermore, in an open-label therapy, a combination of tocilizumab (8 mg/kg up to 800 mg) and anakinra (100–300 mg) were given to thirty-one patients with severe COVID-19 pneumonia, in which one patient infected with candidal infection ( 105 ).…”
