Anti-vascular endothelial growth factors (VEGF) agents have revolutionized the treatment of retinal diseases. Use of anti-VEGF agents in the Indian Scenario present some unique challenges considering the absence of compounding pharmacies, poor penetrance of health insurance and limited affordability of the citizens of a developing economy. To study the changing paradigms of anti-VEGF use in the Indian scenario, all articles published by Indian authors, data from web-based surveys amongst Indian vitreo-retinal specialists were reviewed. In the paucity of compounding pharmacies in India, fractionation and injection techniques differ from those of developed countries. Frequent anti-VEGF monotherapy offers the best anatomical and visual results, but economics of scale do not allow the same in the Indian scenario, resulting in PRN dosing and combination of anti-VEGF with laser photocoagulation, being the commonly employed treatment protocols.

BackgroundRetinal angiomatous proliferation (RAP) has been known as a variant of exudative age-related macular degeneration (AMD) with a unfavorable prognosis. To evaluate the effect of ranibizumab administered initially as three loading doses for patients with various stages of RAP.MethodsA retrospective chart review of 40 patients (41 eyes) with RAP was conducted. The study divided patients into three groups of Group I (8 eyes in stage I), Group II (17eyes in stage II), and Group III (16 eyes in stage III). All patients received three initial monthly intravitreal injections (0.5 mg) of ranibizumab and were monitored monthly for 12 months. Reinjection of ranibizumab after three initial monthly doses was administered on as-needed basis. The main outcome measures were the change in the mean of best-corrected Snellen visual acuity (BCVA) and central macular thickness (CMT), and the total number of injections received during the 12 months.ResultsThe mean change in BCVA at 12 months was-0.286,-0.165, and-0.151 (logMAR) in Group I, II, and III, respectively. CMT was also reduced by a mean of 32.72 ± 56.75, 57.45 ± 56.48 and 148.37 ± 98.59 μm. The mean number of injections in Group I was significant lower than those in Group II and III (P < 0.001, P < 0.001, and P = 0.15 for Group I versus Group II, Group I versus Group III, and Group II versus Group III, respectively).ConclusionsThe 12-month follow-up outcomes suggest that three consecutive loading doses of intravitreal ranibizumab is an effective treatment on early stage (stage I) of RAP. Patients in stage I showed a significantly lower recurrence rate than patients in later stages.

Age-related macular degeneration (AMD) accounts for 8.7% of the global blindness and neovascular form of AMD (nAMD) occupies a large proportion of severe visual loss and legal blindness caused by AMD with a relatively low incidence rate. Choroidal neovascularization (CNV) is overwhelmingly responsible for the occurrence of nAMD as bleeding and fluid leakage followed by abnormal formation of blood vessels could directly lead to loss of central vision so that reduce the choroidal angiogenesis is an ideal treatment method of nAMD. VEGF is an important cytokine which promote the signaling pathway of angiogenesis and the abnormal expression of VEGF is verified in great many CNV cases. Several anti-VEGF drugs have been widely used in clinical treatments such as ranibizumab, bevacizumab and aflibercept. Conbercept, as an originally developed drug in China, has attracted great attention. For the purpose of better treatment efficacy, our group designed a short chain peptide (Sequence: DDIIIRH-NH2, M.W.880.99) for controlled drug release to remedy the drawback of the short half-time period. The peptide could self-assembled into a stable ‘hydrogel under pH 7.4 condition and the 3D structure was clearly observed in TEM study. Rheological study exhibited its great injectability so that the hydrogel was a material for intravitreal injection. Statistics exhibited that the hydrogel could release approximately 50% of total conbercept. The In vitro experiments showed that either dose-dependent or the time-dependent incubation with peptide would not decrease the cell viability of HREC, revealing that the peptide was biocompatible. The most important is that co-incubation with HREC obviously reduced the HREC proliferation and tube formation induced by VEGF, ensuring its potential for the treatment efficacy of nAMD.
