“To Fix a Broken Heart”: An Unusual Case of Infective Endocarditis Involving the Mitral Valve With Perforation and Hemodynamic Instability

Abstract: Infective endocarditis (IE), commonly caused by Staphylococcus aureus, can affect multiple cardiac structures and lead to significant morbidity and mortality. We present a case of IE with extensive mitral valve involvement causing perforation and hemodynamic compromise.A 66-year-old Caucasian female presented to the emergency department for progressive altered mental status and lethargy. The patient and family denied history of intravenous drug use (IVDU) on interview. Physical exam revealed tachypnea, tachyca… Show more

“…As a result of an infection entering the bloodstream, traveling to the heart, and infiltrating coronary tissue, infectious endocarditis (IE) is mostly caused by Staphylococcus aureus [ 1 ]. The possible entrance points are dental work, urinary tract infections, lung infections, gastrointestinal infections, skin conditions, intravenous drug use, surgery, and intravenous cannulation from hospital admissions [ 2 ].…”

Around 5% of the general population is at risk for developing IE, making it a prevalent illness [ 2 ].…”

“…The most common causative agent is Staphylococcus aureus, which can lead to complications such as emboli and abscesses in distant organs such as the central nervous system, spleen, kidneys, or skin. IE has a high mortality and morbidity rate due to the risk of embolic phenomena [ 1 , 2 ].…”
