Background and Aim: Few comprehensive epidemiological studies of the prevalence of tinnitus have been undertaken, and none has been carried out in Egypt. A community-based survey was conducted in the Assiut Governorate to estimate the prevalence of tinnitus, its associations with psychiatric disorders and its effect on the quality of life. Material and Methods: The study involved 8,484 subjects, 5,783 (68.2%) from the rural community and 2,701 (31.8%) from the urban community. Patients were identified from a door-to-door survey and evaluated using a semistructured questionnaire, the Tinnitus Handicap Inventory and the Hamilton Anxiety and Depression Scales. Results: Four hundred and thirty-nine patients with tinnitus were found, giving a prevalence of 5.17 tinnitus cases/100 inhabitants. Males and females were equally affected. The highest age-specific prevalence rate was recorded among subjects above 60 years (17.66%) and was significantly higher among urban than rural inhabitants (6.3 vs. 4.6%) and among illiterate than among educated persons (10.15 vs. 3.07%). A majority of patients (53.3%) had some hearing loss; otitis media was common. Only 15.2% of cases were classified as having severe to catastrophic tinnitus. Nearly two thirds had a degree of depression. There were significant correlations between the severity of tinnitus and the degree of hearing loss, temporomandibular joint pain and the Hamilton depression score. Sleep disturbance was reported by 39.4% of subjects. Life enjoyment was severely affected in 15% of tinnitus patients. Conclusion: Tinnitus is a common problem in our locality, especially in older adults, and is associated with some modifiable risk factors.

Abstract-The Department of Veterans Affairs (VA) considers tinnitus a disability. Veterans can claim tinnitus as a "serviceconnected" disability if the tinnitus is thought to be connected to military service. The VA adjudicates each claim and determines whether reasonable evidence exists to support it. Currently, determining the presence of tinnitus is based on subjective reporting-objective measures do not exist. The aim of this study was to develop and document a test for detecting the presence/absence of tinnitus with high confidence. Using our computer-automated, self-guided tinnitus evaluation system, we conducted three phases of testing to compare psychoacoustic measures of tinnitus between participants with versus without tinnitus. Phase 1 measures included loudness match, pitch match, minimum masking level, residual inhibition, Békésy, and forced-choice double staircase. Phases 2 and 3 measures were chosen based on results of the previous phase. The number of tests and time of testing decreased during each successive phase. Differences were seen between groups; most notably, higher low-frequency loudness matches and higher median pitch matches were observed for participants with tinnitus. Results of this study suggest that further efforts can produce a defined psychoacoustic test battery for identifying the presence/absence of tinnitus.

One of the most criticized points in tinnitus clinical studies arise from the lack of consensus about measurement methods. Aim:To evaluate the correlation between audiometric thresholds, pitch matching (PM), minimum masking level (MML), Tinnitus Handicap Inventory (THI) and the Beck Depression Inventory (BDI) in tinnitus patients. Study design:Prospective, cross-sectional. Materials and methods:Subjects were submitted to tonal audiometry, PM and MML for tinnitus. They also filled out the THI and BDI. Data was statistically compared for correlation purposes between audiometric thresholds, psycho-acoustic measures and questionnaires.Results: There was no statistically significant correlation between THI and MML, both in patients with BDI scores under and over 14 points. There was no statistically significant correlation between the worst hearing frequency and PM, as well as between the cut-off frequency and the PM in patients with descending hearing curves in their audiograms. Conclusions:There is no statistically significant correlation between psycho-acoustic measures (PM and MML), audiometric thresholds, THI and BDI. Tinnitus is a very complex symptom and isolated measures by psycho-acoustic methods; tinnitus and depression questionnaires are not satisfactory. Braz J Otorhinolaryngol. 2010;76(4):522-6.
