Management of Tinnitus - The Enriching Views of Treatment Options

Tinnitus is defined as a sound a person hears that is generated by the body, rather than by outside source. The word tinnitus is derived from the Latin "tinnire" meaning "to ring" and is perceived as ringing, buzzing, or hissing in or around the ear(s). Approximately 50 million Americans are affected, while there is a prevalence of 10% in the United Kingdom among adult population. It has multiple etiologies and is sometimes idiopathic. Tinnitus may vary widely to pitch, loudness, description of sound, special localization, and temporal pattern. Most often, tinnitus is associated with other aural symptoms, such as hearing loss and hyperacusis. Tinnitus may result in sleep disturbances, work impairments, distress. Males are more likely to suffer from tinnitus. In the mechanically demanding and biochemically active environment of the temporomandibular joint (TMJ), therapeutic approaches are capable of restoring joint functionality. TMJ treatments including splints, occlusal adjustments, and jaw exercises have been shown to be more effective than no treatment. The following chapter presents a synopsis of etiology, current treatment methods, and the future of tissue engineering for repairing and/or replacing diseased joint components, specifically the mandibular condyle and TMJ disc.

Introduction Excessive caffeine intake has been thought to be a contributory factor for tinnitus. However, there has been no systematic review to elucidate the causal relationship between caffeine intake and the incidence of tinnitus. Objectives We performed the current review aiming at evaluating the evidence from the current literature for the relationship between caffeine intake and the incidence of tinnitus. Data Synthesis Databases including PubMed, Scopus, and Google scholar were searched for relevant articles. A total of 142 studies were screened for eligibility, of which four articles met our inclusion criteria: two were prospective cohorts and two were cross-sectional studies. Although one study found no association between caffeine consumption and the incidence of tinnitus, an inverse relationship was reported by two population-based studies. Concerning patients with preexisting tinnitus, reduction of caffeine intake in a subset who consumed 150 ml to 300 ml/day of coffee yielded a favorable outcome in tinnitus severity. However, those with higher dose intake were less prone to have improvement in the severity of tinnitus. Conclusion Although the current review was inconclusive, it appears that the incidence of tinnitus in previously unaffected individuals might be prevented by a high dose of caffeine intake. However, in preexisting tinnitus, a high dose of caffeine may adversely interfere with the efficacy of caffeine reduction.

Este trabalho objetivou realizar uma análise do discurso do profissional fonoaudiólogo com base na teoria dialógica durante o atendimento de pacientes com zumbido utilizando o Cuidado Centrado na Pessoa. Temos em vista a construção da Interação discursiva (VOLÓCHINOV, 2017) a partir do momento que a profissional se utiliza do Cuidado Centrado na Pessoa. Nesse sentido, nossa pretensão foi averiguar como os sentidos se concretizam no diálogo entre a fonoaudióloga e seus pacientes. Os dizeres aqui construídos têm subsídio teórico nos pressupostos de Bakhtin (2006a, 2006b, 2010), Volóchinov (2017) e Medviédev (2016), ao tratarem da problemática do discurso. Em perspectiva dialógico-discursiva, no âmbito das discussões em Ciências Humanas, divide-se este trabalho em três seções. Buscamos, na primeira seção, apresentar considerações basilares sobre o zumbido e como este é tratado por meio do Cuidado Centrado na Pessoa. Na segunda seção, realizamos uma discussão sobre a importância do uso do Cuidado Centrado na Pessoa no atendimento de pacientes com zumbido; enquanto a terceira seção compreende a análise, em que averiguamos, por meio de abordagem dialógica, o discurso da profissional fonoaudióloga sobre zumbido.
