Timing is everything: Impact of combined long bone fracture and major arterial injury on outcomes

Abstract: BACKGROUND Timing of extremity fracture fixation in patients with an associated major vascular injury remains controversial. Some favor temporary fracture fixation before definitive vascular repair to limit potential graft complications. Others advocate immediate revascularization to minimize ischemic time. The purpose of this study was to evaluate the timing of fracture fixation on outcomes in patients with concomitant long bone fracture and major arterial injury. … Show more

“…26 The injured extremity is assessed for external bleeding and changes in pulse quality and capillary refill. 27,28 Doppler examination with ankle/brachial or ankle/ankle index provides a sensitive tool to raise awareness about the likelihood of a significant vascular injury. 28 The index is calculated as the systolic blood pressure measured on the injured extremity divided by the blood pressure of the contralateral uninjured (upper or lower) extremity.…”

“…11 In cases of combined vascular and orthopaedic skeletal injury, debate continues regarding the best timing and order of revascularization and skeletal stabilization. 12,13 It may be beneficial to consider initial temporary shunting before provisional fracture stabilization in certain situations to minimize ischemia time and reperfusion injury and increase distal tissue viability. 14,15 Aggressive debridement should precede skeletal stabilization so that maximum access to the wound facilitates removal of contaminated and devitalized tissues.…”
