DOI: 10.24070/bjvp.1983 AbstractThe aim of the present study was to investigate the effects of intraportal administration of hypothermic hydrogenrich saline solution (HRSS) on hepatic ischemia/reperfusion (I/R) injury. Thirty rats were divided equally into six groups; 1) sham, no I/R or transfusion, 2) I/R injury (60 minutes ischemia + 120 minutes reperfusion, 3) I/R injury + normal saline 24°C, 4) I/R injury + normal saline 4°C, 5) I/R injury + HRSS 24°C, 6) I/R injury + HRSS 4°C. In groups 3-6, 1 mg/kg normal saline (NS) and/or HRSS were administered into the vein of the left lateral and median lobes of the liver (upper the site of clumping) 10 minutes before finishing of ischemic period. The harvest time points were at 2 hours post reperfusion in all groups. Cell death, sinusoidal dilatation, congestion, hemorrhage, and neutrophil infiltration were observed in I/R group, while these histopathological changes were attenuated in the hypothermic HRSS administrated groups (P < 0.01). The level of alanine aminotransferase, aspartate aminotransferase, malondialdehyde, interleukin 6, tumor necrosis factor a, and caspase-3 were increased significantly by I/R injury and hypothermic HRSS administration reduced all these markers (P < 0.01). SOD level was low in I/R group whereas it tended to increase in the hypothermic HRSS administrated groups (P < 0.01). The present study demonstrated that hypothermic hydrogen-rich saline solution effectively protected the hepatic tissue against cellular injury and organ dysfunction through the mechanisms that decrease the effect of oxidative stress, inflammation, apoptosis and necrosis.

São Paulo: Faculdade de Medicina, Universidade de São Paulo; 2010.Introdução: A isquemia-reperfusão hepática é um fenômeno inerente aos procedimentos cirúrgicos sobre o fígado. Descrita pela primeira vez em 1975, apresenta efeitos maléficos tanto locais quanto sistêmicos que aumentam a morbidade e mortalidade dos pacientes. Dentre as formas de se atenuar o processo, a utilização de drogas anti-inflamatórias como a solução salina hipertônica a 7,5% (SSH) e a pentoxifilina vêm sendo testadas com este propósito. As duas substâncias foram utilizadas conjuntamente (HPTX) em modelos experimentais em choque hemorrágico. Contudo, na isquemia-reperfusão de origem hepática, estas foram administradas apenas isoladamente, o que torna o presente estudo pioneiro nesta avaliação. Objetivo: Avaliar os efeitos da solução salina hipertônica a 7,5% associada a pentoxifilina na isquemia-reperfusão hepática. Métodos: Foram utilizados 138 ratos Wistar divididos em quatro grupos de acordo com o tratamento empregado: GCgrupo controle (sem tratamento), SSF -solução salina fisiológica, SSH -solução salina hipertônica a 7,5% e HPTX -solução salina hipertônica a 7,5% associada a pentoxifilina. A isquemia hepática foi realizada seletivamente sobre o pedículo comum do lobo mediano e ântero-lateral esquerdo pelo período de 60 minutos. As soluções foram administradas 15 minutos antes da reperfusão hepática. No sangue, foram analisadas as dosagens de transaminases hepáticas nos períodos de quatro e 12 horas e, interleucina-6 e interleucina-10 no período de 12 horas. No tecido pulmonar foram avaliadas as dosagens de azul de Evans e mieloperoxidase pulmonar nos períodos de quatro e 12 horas e, no tecido hepático do lobo isquêmico e não-isquêmico foram analisadas as dosagens de malondialdeído e a avaliação da respiração mitocondrial no período de quatro horas e a histologia nos períodos de quatro, 12 e 24 horas. Resultados: Os grupos tratados com SSH isolada ou em conjunto com a pentoxifilina apresentaram níveis significantemente menores (p<0,05) nas dosagens de transaminases e interleucina-6 em comparação aos outros dois grupos (GC e SSF). Resultados semelhantes entre os grupos foram observados no tecido pulmonar através da análise do azul de Evans e mieloperoxidase pulmonar em quatro e 12 horas e no tecido hepático com relação à respiração mitocondrial. No grupo no qual se adicionou pentoxifilina, houve diminuição estatisticamente significante da peroxidação lipídica e da permeabilidade pulmonar tardia quando comparado ao grupo SSH. Conclusões: A utilização em conjunto das duas soluções reduziu: a resposta inflamatória sistêmica; as lesões histológicas e funcionais do fígado; o estresse oxidativo e a permeabilidade pulmonar. Liver ischemia–reperfusion (IR) injury is associated with poor outcome after liver transplantation and liver resections. Hexafluoroisopropanol (HFIP) is a tri‐fluorinated metabolites of volatile anesthetics and has modulatory effects on inflammation that have been observed mainly in cell culture experiments. In this survey, we investigated the effects of HFIP in a rat model of normothermic hepatic ischemia–reperfusion injury. Twenty‐four male Wistar rats were randomized into three groups: (1) control in which animals were submitted to 30 min of partial liver ischemia with resection of non‐ischemic liver lobes immediate after reperfusion, (2) pre‐ischemia (PI) group in which animals received intravenous HFIP (67 mg/kg) 5 min before liver ischemia, and (3) pre‐reperfusion (PR) group in which animals received intravenous HFIP (67 mg/kg) 5 min before reperfusion. Four hours after reperfusion, all animals were euthanized for sample collection. Aspartate and alanine transaminases, glucose, and high mobility group box‐1 (HMGB‐1) protein concentrations showed a significant decreased, and malondialdehyde was increased in the PR group compared with control and PI groups. Interleukin 6 (IL‐6) was increased in the PI group compared with control and PR groups. IL‐10 and ‐12 were increased in the PR and PI groups, respectively, when compared with the control group. Glucose decreased in the PR when compared with the control group. Post‐conditioning with HFIP led to a decrease in hepatocellular injury and was associated with a downregulation of HMGB‐1. The HFIP resulted in a better control of inflammatory response to ischemia–reperfusion even without causing a reduction in oxidative stress.
