Time-trend and recurrence analysis of pancreatic neuroendocrine tumors

Abstract: This study aimed to evaluate the evolving trends in clinicopathological features of pancreatic neuroendocrine tumors and to analyze the predictors of recurrence after curative resection. Data collected retrospectively from a single center between January 1990 and December 2017 were analyzed. Patients were categorized chronologically into three groups for evolving time-trend analysis. Overall, 542 patients (300 female, 55.4%) underwent surgical resection for pancreatic neuroendocrine tumors, including 435 (80.3… Show more

“…However, there are several retrospective small cohort studies that suggest the contrary. 20 , 26 , 27 , 28 , 29 Interestingly, in a study done by US Neuroendocrine Tumor Study Group investigating 873 patients with nonfunctional localized (no distant metastatic disease) PNET that underwent surgical resection, recurrence-free survival was found to be associated with a multitude of factors, including tumor burden and lymph node involvement but not vascular involvement. 26 The 5-year disease-free survival in patients found to have locally advanced tumor was 60%, which was similar to our findings.…”

“…Non-functional PanNETs, due to the lack of early symptoms, are normally detected at later stages with locally advanced or metastatic disease. Non-functional PanNETs are a more heterogeneous group of tumours with unpredictable and varying degrees of malignancy 1 , 10 . The best predictor of clinical outcome of PanNETs is the fraction of proliferating neoplastic cells, with high-grade (G3) tumours being more aggressive disease and with molecular alterations that often align them with neuroendocrine carcinomas 1 .…”

“…Prognosis depends on the histological findings and the presence of metastasis. Also, regardless of primary lesion resection, and control of secreted hormone levels and clinical manifestations, recurrence rate is high, particularly, with liver metastases as in the case of our patient ( 35 ).…”
