Aims: This systematic review aimed to compare the effectiveness of sodium hypochlorite and chlorhexidine for root canal disinfection during root canal therapy. Methods:A literature search for clinical trials was made on the PUBMED (Medline), Web of Knowledge, SCOPUS, and Science Direct databases and in the reference lists of the identified articles up to January 2015. Quality assessment of the selected studies was carried out according to the CONSORT statement. Results:One clinical trial and four randomized clinical trials (RCTs) were selected from the 172 papers initially identified. There was heterogeneity in the laboratory methods used to assess the root canal disinfection as well as in the concentrations of the irrigants used. Therefore, meta-analysis was not performed. Two studies reported effective and similar reductions in bacterial levels for both irrigants. Sodium hypochlorite was more effective than chlorhexidine to reduce microorganisms in one study and another reported opposite findings. Both root irrigants were ineffective in eliminating endotoxins from necrotic pulp root canals in one study. Trial design and information regarding randomization procedures were not clearly described in the clinical trials. No study compared laboratory results with clinical outcomes. Conclusions:The available evidence on this topic is scarce and the findings of studies were not consistent. Additional RCTs using clinical outcomes to compare the use of sodium hypochlorite and chlorhexidine during root canal therapy are needed.

Ex vivo evaluation of three instrumentation techniques on E. faecalis biofilm within oval shaped root canals Abstract: The objective of the present study was to assess the effectiveness of reciprocating instrumentation in disinfecting oval-shaped root canals infected with Enterococcus faecalis. Forty-five human lower premolars were infected with a culture of E. faecalis (ATCC 29212) for 28 days. Five other teeth that were neither contaminated nor instrumented were used as controls. The 45 specimens were divided into three experimental groups (n = 15) based on the root canal preparation technique used: manual (K-type, Dentsply Maillefer, Ballaigues, Switzerland); rotary (MTwo, VDW GmbH, Munich, Germany); and reciprocating (Reciproc R50, VDW GmbH, Munich, Germany) instruments. During chemomechanical preparation, 21 mL of 2.5% NaOCl was used as the irrigating solution. Microbiological sampling was performed before (S1) and immediately after (S2) the chemomechanical preparation using sterilized paper points. Specimens were then cleaved, and 0.02 g of dentine chips was collected from the root thirds to verify the presence of microorganisms in dentinal tubules. All three preparation techniques reduced the number of microorganisms in the root canal lumen and dentine chips from the root thirds, but no significant differences were observed between the three groups (p > 0.05). Reciprocating instrumentation with Reciproc R50 was effective in reducing the number of microorganisms within the root canal system. Although this technique involves the use of only one file to perform the root canal therapy, it is as effective as conventional rotary instrumentation in reducing the E. faecalis biofilm from the root canal system. However, further clinical investigations are warranted in order to ratify these results.

Objective: The aim of this study was to investigate the effects of two different self-adjusting file (SAF) working times and three different chelating agents on the push-out bond strength of root canal fillings in oval root canals. Materials and Methods: Eighty mandibular premolars were selected and distributed into 8 groups (n=10): 4 min SAF (G1), 4 min SAF + 17% ethylenediaminetetraacetic acid (EDTA) (G2), 4 min SAF + 10% citric acid (CA) (G3), 4 min SAF + 1% peracetic acid (PAA) (G4), 6 min SAF (G5), 6 min SAF + 17% EDTA (G6), 6 min SAF + 10% CA (G7), and 6 min SAF + 1% PAA (G8). The root canals were filled using guttapercha and AH Plus root canal sealer, and the teeth were prepared for the push-out assessments using 1.0 mm thick root slices. Loading was performed with a universal testing machine at a speed of 0.5 mm/min; and then, collected data were analyzed statistically (α=0.05). Results: While the different chelating agents significantly affected the push-out bond strength of the root canal filling (p<0.05), the different SAF working times did not (p>0.05). There were no significant differences among the groups in the apical and middle thirds of the roots (p>0.05). In the coronal thirds, G6 and G8 indicated significantly greater bond strength values than G1 and G5 (p<0.05). Conclusion: According to the results, the 4 and 6 min SAF working times did not affect the bond strength of the root canal filling. PAA and EDTA can be recommended for root canal irrigation after SAF usage.
