Time course of reverse remodeling of the left ventricle during support with a left ventricular assist device

Abstract: Maximum structural reverse remodeling by left ventricular assist devices is complete by about 40 days. Molecular reverse remodeling of sarcoplasmic endoreticular calcium adenosine triphosphatase 2a expression is quicker, being complete by about 20 days.

“…Furthermore, LVADs have been shown to lead to partial normalization of myocardial structure and function, including increased ␤-AR responsiveness (30); decreased myocyte volume (40), width (41), and diameter (22); and improved myocyte contractility (10) in a process termed "reverse remodeling." We were the first to report both global and etiology-specific differential gene expression using paired tissue samples from patients in end-stage HF and following LVAD-mediated reverse remodeling, followed by several other reports (4,5,15,24).…”

“…7,13,[20][21][22] In this study, the LVEF increased significantly during PF LVAD support compared with CF devices. Although clinical backgrounds were not necessarily matched completely, some authors showed more volume unloading with PF devices, 13,14 while others showed that a PF pump was better than a CF pump considering the improvement in LVEF or explantation in the clinical setting.…”

“…Usually, left ventricular assist devices are employed as a bridge to heart transplant [6]. It follows that they should be discontinued once one is no longer a candidate for transplant.…”
