Time and Mode of Delivery in Twin Pregnancies

Abstract: There are many suitable recommendations for twin gestation term in the literature. In many protocols, resolution is recommended for dichorionic pregnancies around 38 weeks, at 36 weeks for monochorionic (devoid of complications) and at 32-34 weeks in cases of single amniotic chamber. The main risk associated with vaginal delivery is connected to the possibility of anoxia of the second twin. However, a cesarean delivery performed by non-cephalic presentation of the second twin is associated with increased mater… Show more

“…Delayed childbearing, higher birth orders, and advancements to medically assisted reproduction are widely accepted as reasons for the increasing rates 1 . Twin births are more likely to have complications during birth, to be born prematurely 1–3 and to be born by cesarean section (CS) 3,4 . Additionally, twin births pose a significantly higher risk for maternal complications, including pre‐eclampsia 5 and postpartum hemorrhage 1–3,6 …”

“…Many important factors are involved when determining the delivery method, including women's parity, foetal weight, foetal presentation, inter-twin weight discordance and maternal clinical conditions 6 . Some studies have explained that if both foetuses are in a cephalic presentation, the vaginal mode is preferred 7 . Vaginal delivery is possible if the foetus weighs more than 1500 grams.…”

“…7 Several studies have recommended dichorionic pregnancies around 38 weeks, at 36 weeks for monochorionic (devoid of complications), and 32-34 weeks in cases of single amniotic. 8 The main causes of adverse neonatal outcomes in multiple pregnancies are related to prematurity, fetal growth restriction, and low birth weight.…”
