Tilt and Rotation Correction of Acetabular Version on Pelvic Radiographs

Abstract: Anteroposterior pelvic radiographs are the gold standard of imaging for mechanical hip problems. However, correct interpretation is difficult because the projected morphologic features of the acetabulum and nearly all routinely used hip parameters depend on individual pelvic position, which can vary considerably during acquisition. We developed software that recreates the projected acetabular rim and the measured hip parameters as if obtained in a standardized orientation. The vertical and horizontal distances… Show more

“…During this study period, all patients younger than 60 years who were evaluated for hip pain when the diagnosis of fracture was not being considered received supine and standing radiographs and formed the basis of this retrospective study. Patients were excluded if the sacrococcygeal joint or coccyx tip was not observed owing to a lead shield or bowel gas obstruction (n = 31), if they had arthritis of Tönnis grade 2 or greater (n = 26), malrotation greater than 5 mm [5,11] (n = 24), previous hip or proximal femoral surgery (n = 5), or previous lumbar fusion (n = 2). Malrotation was measured by drawing a vertical line through the center of the symphysis and the sacrum and measuring the distance between these two.…”

“…Mixed-type FAI had features of both cam-and pincer-type of FAI on conventional radiographs (Table 1). A contraindication for surgical treatment during the time of this study was advanced OA, defined as a Tönnis grade [45] of III. Preoperative clinical evaluation was performed by different observers and included the patient history, anterior impingement test [44], and full goniometric ROM.…”

“…As such, their technique is unable to provide quantitative assessment of the actual amount of crossover. Other techniques do not provide accurate measurements of the length and width of crossover [3,4,23]. Our proposed technique provides consistent, quantitative measurement of the length and width of crossover, thereby defining the amount of overhanging acetabular rim.…”

“…We defined the inclination of the pelvis according to the method of Tannast et al [23]. For consistency, neutral position for each pelvis was defined at an angle of 60°between the sacral promontory and the pubic symphysis.…”
