Tilt-adjusted Cup Anteversion in Patients with Severe Backward Pelvic Tilt is Associated with the Risk of Iliopsoas Impingement: A Three-dimensional Implantation Simulation

Abstract: Background Anterior overhang of the acetabular component is associated with iliopsoas impingement, which may cause groin pain and functional limitations after THA. However, little is known about the relationship between component overhang and functional alignment of the acetabular component. CT-based image simulation may be illuminating in learning more about this because CT images are more effective than radiographs for evaluating the component’s overhang and position. … Show more

“…The position of the cup after THA correlates with the risk of postoperative iliopsoas impingement [ 35 ]. Inclination and anteversion of the cup in the present study was found to be 44,6° (31–52) and 18,7° (11–33), respectively, which is in line with previous literature [ 22 , 35 ]. One patient in our study had an inclination over 50° (52) and this patient later underwent revision surgery.…”

“…In a simulator study the acetabular cup overhang was more severe when the pelvis tilted posteriorly and posterior PSI can be a risk factor for IPI too. 17 Our patients with posterior PSI were significantly older ( p < 0.001) and tended to have a lower incidence of symptomatic IPI ( p = 0.650) than the control group, despite a larger cup protrusion length ( p = 0.058). In general, the posterior PSI angle becomes larger with age, and it is common for older patients to have spinal diseases, which lead to a forward bending posture and decreasing lumbar lordosis.…”

“…A CT simulation study found that large posterior pelvic sagittal inclination (PSI) was associated with anterior overhang of the cup. 17 Therefore, we hypothesised that posterior PSI may also be important. However, there is limited clinical literature on the association of pelvic inclination with symptomatic IPI.…”

“…Medical records were retrospectively reviewed to collect demographic data, including age, sex, height, and weight at the time of CT. Pelvic CT (LightSpeed VCT system; GE Medical Systems, Chicago, IL; slice thickness, 1 mm; resolution, 300 dpi) was performed in the supine position. Images were obtained in Digital Imaging and Communications in Medicine format and digitally reconstructed radiographs (DRRs) and multiplanar reconstruction images created using ZedHip software (Lexi, Tokyo, Japan) 22‐26 . The pelvic position was standardized with reference to the APP coordinate system 1 .…”
