Hypothesis. Cancer patients frequently suffer from poor sleep quality. Bryophyllum pinnatum is a herbal medication used in anthroposophic medicine, which has been shown to be associated with improvements in sleep quality during pregnancy with only few and minor or moderate side-effects reported. In this study, the sleep quality of cancer patients during treatment with B pinnatum was investigated. Study Design. In this prospective, observational study, cancer patients suffering from sleep problems were treated with B pinnatum (350 mg tablets, corresponding to 50% of leaf pressed juice [Weleda AG, Arlesheim, Switzerland], dosage at physician’s consideration, but most frequently 2 tablets with evening meal and 2 before going to bed). Methods. Sleep quality (Pittsburgh Sleep Quality Index [PSQI]), daily sleepiness (Epworth Sleeping Scale [ESS]), and fatigue (Fatigue Severity Scale [FSS]) were assessed at the beginning of the treatment and after 3 weeks. Possible adverse drug reactions perceived by the patients during the treatment were recorded. From the 28 recruited patients, 20 completed both questionnaires and were considered in the present analysis. Data are expressed as mean ± standard deviation. Results. Patients were 61 ± 10.4 years old and the majority were female (17 out of 20). During treatment with B pinnatum, the PSQI decreased from 12.2 ± 3.62 to 9.1 ± 3.61 (P < .01), and ESS changed from 8.4 ± 3.18 to 7.1 ± 3.98 (P < .05). There was no change in FSS. The treatment was well tolerated by the majority of patients, with only 6 patients reporting discomfort that might have been caused by B pinnatum (fatigue n = 3, dry throat n = 1, agitation n = 1, difficult digestion n = 1). No serious adverse drug reactions were detected. Conclusion. B pinnatum may be a suitable treatment for sleep problems of cancer patients. Controlled, randomized clinical trials of the use of B pinnatum in sleep disorders are urgently needed.

Essential oils (EOs) and their individual volatile organic constituents have been an inherent part of our civilization for thousands of years. They are widely used as fragrances in perfumes and cosmetics and contribute to a healthy diet, but also act as active ingredients of pharmaceutical products. Their antibacterial, antiviral, and anti-inflammatory properties have qualified EOs early on for both, the causal and symptomatic therapy of a number of diseases, but also for prevention. Obtained from natural, mostly plant materials, EOs constitute a typical example of a multicomponent mixture (more than one constituent substances, MOCS) with up to several hundreds of individual compounds, which in a sophisticated composition make up the property of a particular complete EO. The integrative use of EOs as MOCS will play a major role in human and veterinary medicine now and in the future and is already widely used in some cases, e.g., in aromatherapy for the treatment of psychosomatic complaints, for inhalation in the treatment of respiratory diseases, or topically administered to manage adverse skin diseases. The diversity of molecules with different functionalities exhibits a broad range of multiple physical and chemical properties, which are the base of their multi-target activity as opposed to single isolated compounds. Whether and how such a broad-spectrum effect is reflected in natural mixtures and which kind of pharmacological potential they provide will be considered in the context of ONE Health in more detail in this review.

Diabetes mellitus Typ 2 hat aufgrund seiner hohen Prävalenz, seines chronischen Verlaufs und seiner Folgeerkrankungen eine grosse medizinische, aber auch eine sozioökonomische Bedeutung. Es handelt sich um eine komplexe Erkrankung mit einer Vielzahl an pathogenen Faktoren, wobei neben der metabolischen Störung vor allem auch chronisch entzündliche Prozesse beteiligt sind. Oxidativer Stress und die Bildung von Advanced Glycation Endproducts (AGE) spielen eine zentrale Rolle bei der Entstehung und dem Fortschreiten diabetischer Folgeerkrankungen, wie z.B. mikro- und makrovaskulärer Störungen, akzelerierter Arteriosklerose, diabetischer Neuro-, Nephro- und Retinopathie sowie anderer assoziierter chronisch entzündlicher Erkrankungen. Die pathogenen Mechanismen sind komplex; sie beinhalten redundante Signalwege sowie positive Rückkoppelungen und verstärken sich gegenseitig. Daher sind zur Behandlung und Prävention vor allem Arzneimittel geeignet, die ebenfalls ein netzwerkartiges, pleiotropes Wirkmuster haben und an mehreren Stellen im Krankheitskomplex angreifen. Phytotherapeutika besitzen in der Regel eine solche komplexe Wirksignatur. Ein Beispiel für ein solches Phytotherapeutikum mit komplexer Signatur ist das Arzneimittel Padma® 28, das in der Schweiz nach den Traditionen der Tibetischen Medizin hergestellt wird. In der vorliegenden Arbeit werden die Wirkmechanismen dieses Vielstoffgemischs in den Kontext der pathogenen Vorgänge des Diabetes mellitus und seiner Folgeerkrankungen gestellt und diskutiert. Unter diesen scheinen vor allem vielfältige direkte und indirekte antioxidative Effekte, die Hemmung der Bildung von AGE und Advanced Oxidation Protein Products (AOPP) sowie die antientzündliche Aktivität im Vordergrund zu stehen. Das Wirkprofil von Padma 28 scheint damit über arterielle Durchblutungsstörungen hinaus auch die bei Diabetes verstärkten und beschleunigten pathogenen Prozesse günstig beeinflussen zu können. Die Resultate weisen auf einen möglichen Nutzen des Präparats auch bei Diabetes-bedingten vaskulären Störungen und assoziierten chronisch entzündlichen Erkrankungen hin.
