Thyroid Storm in a Patient With COVID-19

Abstract: Objective Thyroid storm is a severe exacerbation of thyrotoxicosis that can cause significant morbidity and mortality. The emergence of the novel coronavirus (SARS-CoV-2) that originated in Wuhan, China has become a worldwide pandemic. We present the first documented case of thyroid storm (as defined by the Burch-Wartofsky criteria) in a patient positive for COVID-19. Methods Laboratory and diagnostic studies including thyroid function tests, thyroid antibody testing, S… Show more

“…Some specific clinical characteristics that strengthen the suspicion of a patient having a thyroid crisis are high fever, tachycardia, nausea, agitation and changes in mental status. In patients with suspected thyroid crisis, assess with the Burch-Wartosky table to calculate the clinical index score for thyroid crisis (Milani et al, 2021;Rao et al, 2021). Thyroid storm requires immediate and aggressive emergency medical attention.…”

“…The thyroid function test showed a thyroid stimulating hormone (TSH) level of < 0.01 mIU/L [3 rd trimester-specific reference range (3TRR): 0.38-4.04 mIU/L[ 6 ]], free T4 level of 1.95 ng/dL (3TRR: 0.5-0.8 ng/dL[ 6 ]), and total T3 level of 183.9 ng/dL (3TRR: 123-162 ng/dL[ 6 ]), which reflect overt hyperthyroidism. According to the Burch-Wartofsky Point Scale (BWPS)[ 2 ], the score was 65, which was highly suggestive of a TS.…”

“…Laboratory results demonstrated very low TSH levels (TSH<0.01 mIU/L) and high levels of FT4 (5.34 ng/dL) and TT3 (654 ng/dL). Thyroid ultrasound revealed heterogeneous echotexture with increased vascularity ( 158 ).…”
