Thyroid nodule surgery: Predictive diagnostic value of fine-needle aspiration cytology and frozen section

Abstract: FNAC and frozen section have a comparable predictive diagnostic accuracy. Frozen section is requested by the surgeon not only on the basis of preoperative FNAC, especially when it is suspicious, or even indeterminate, but also in the light of the macroscopic surgical findings.

“…Affecting 4%–7% of the general population, palpable thyroid nodules are common. 1 The incidence of thyroid cancer has nearly tripled from 4.9 to 14.3 per 100,000 individuals over a thirty-five year period ending in 2009. 2 Such a high incidence mandates a cost-effective and efficient method for the preoperative evaluation of thyroid nodules.…”

“…One of the reason for low sensivity rate of FNAC may be due to incidental papillary micro carcinomas as pointed out by many authors [17,19] either due to inadequate sampling , or error in interpreta on [2,17]. One case was diagnosed as colloid goitre with papillary hyperplasia on FNAC as the nuclear features typical of papillary carcinoma were lacking in this par cular case.…”

“…The sensitivity and specificity of frozen sections for thyroid lesions or extrathyroidal extension were 58%–94% and 93%–100%, respectively. 5 , 20 22 Advantages of using intraoperative frozen sections were the speed of processing and the high rate of correct diagnoses, but FN diagnoses continued to exist. FN diagnoses may result in reoperation, which has a higher risk of permanent hypoparathyroidism and recurrent laryngeal nerve injury.…”
