Thyroglobulin Measurement in Fine-Needle Aspiration Improves the Diagnosis of Cervical Lymph Node Metastases in Papillary Thyroid Carcinoma

Abstract: For diagnosing cervical lymph node metastases in PTC, FNA-Tg is a valuable adjunct to FNAC. Its use should be considered to avoid missing metastatic disease in patients who may benefit from lymphadenectomy.

“…Cervical lymph node metastasis in patients with PTC is closely related to the prognosis. Accurate diagnosis of metastatic cervical lymph nodes before surgery is essential[9-11]. Currently, ultrasound is commonly used in the diagnosis of PTC cervical lymph node metastasis[12-14].…”

“…The value of FNA in the diagnosis of lymph node metastases has been widely established in previous studies . In our study, it was possible to perform a preliminary qualitative diagnosis in most cases of metastatic NPC on the basis of distinctly shaped cells, where a high sensitivity was achieved by observing FNA cell smears (86.8%).…”

“…Accordingly, EBERs play a helpful role in the diagnosis of NPC. Some of the histomorphological features of NPC15 can partially be observed in lymph node metastases, which serves as evidence in smears. The value of FNA in the diagnosis of lymph node metastases has been widely established in previous studies [18][19][20][21][22][23]. “…However, US has several limitations, such as low sensitivity and operator dependency (2). Cytology after US-guided FNA of the cervical LNs has high specificity, but a 5–10% inadequacy rate and a 6–50% false-negative rate, especially in LNs with small size or cystic change (3, 1416). In our study, 10 patients (29.4%, 10/34) had false-negative cytology, and among them, 9 (90%) showed cystic change of the LNs on US.…”
