Thrombus Aspiration Complicated by Systemic Embolization in Patients With Acute Myocardial Infarction

Abstract: ercutaneous coronary intervention (PCI) is now the standard treatment for acute coronary syndrome (ACS). The optimal outcome of reperfusion treatment includes not only sustained patency of the epicardial coronary artery, but also optimal reperfusion of the myocardium throughout the microvasculature. It has been reported that in patients treated with primary PCI, distal embolization may be visualized on the coronary angiogram of 15.2% of such patients. 1 Embolization may occur spontaneously following plaque rup… Show more

“…Thrombus aspiration may not be a risk-free procedure. Systemic embolization can occur, 26 and in a recent meta-analysis, thrombus aspiration was associated with a trend toward an increased rate of stroke (P = 0.06). 14 However, in the TASTE trial, there was no significant difference between the groups in the rates of stroke and neurologic complications.…”

“…not significant Firstly, concerns exist about safety as thrombectomy itself is not a risk-free procedure. Systemic embolization has been described [33], and in a recent meta-analysis, thrombus aspiration was associated with a trend toward an increased rate of stroke [23]. However, in the TASTE trial no significant differences between the groups in the rates of stroke and neurologic complications were reported [34].…”

“…To the best of our knowledge, this is the first report of the successful retrieval of BioGlue leading to coronary embolism using a thrombectomy catheter in which the material was confirmed to be BioGlue based on histologic examinations conducted during the patient's life. The use of aspirating coronary embolic materials with a thrombectomy catheter has been reported, whereas this procedure can potentially be complicated by systemic embolism [7]. Therefore, cardiologists must take care not to dislodge the retrieved materials when recovering them from the coronary arteries.…”
