Thrombus aspirated from patients with ST-elevation myocardial infarction: Clinical and angiographic outcomes

Abstract: ObjectiveTo investigate differences in clinical and angiographic outcomes between patients with acute myocardial infarction with red and white thrombi.MethodsA total of 137 patients with ST-segment elevation myocardial infarction undergoing primary percutaneous coronary interventions were included. Thrombus material was classified as white or red based on its pathology. Information on characteristics of thrombi was available for 97 (70.8%) patients.ResultsThe total ischaemic time was significantly longer in th… Show more

“…With this case report, we have described a STEMI case with white clot extraction. STEMIs are usually associated with red clots, whereas NSTEMIs have been known to be associated with white clots [ 12 ]. Despite receiving rTPA at the initial point-of-care, the patient still presented with an ongoing STEMI, with positive EKG findings, elevated high-sensitivity troponins, and active chest pain.…”

“…The atheroma in patients with STEMI, red thrombus, is mainly formed by fibrous protein aggregation, which is suitably treated using medications for thrombolysis. On the contrary, the atheroma in patients with NSTEMI, white thrombus, is mainly formed by blood platelet aggregation [ 9 , 10 , 11 , 12 , 13 ]. These two forms of thrombus have different pathological mechanisms, which should be treated using different methods.…”
