S Levamisole Vasculitis with neovascularisation: 2 case reportsTwo men developed vasculitis with neovascularisation after using cocaine adulterated levamisole [dosages, routes and duration of treatment to reaction onsets not stated; not all routes stated].Case 1: A 31-year-old man, presented with recurrent purpuric lesions on both earlobes and neutropenia. His biopsy revealed thrombotic vasculopathy and intravascular papillary endothelial hyperplasia. His lesions completely resolved with prednisone.Case 2: A 38-year-old man had recurrent fever, arthralgias and myalgias with cutaneous lesions after cocaine use. After the last inhalation of cocaine adulterated levamisole, he presented with retiform purpura, stellate atrophic plaques and ulcers on cheeks, earlobes and legs and neutropenia and severe vital organ involvement . His biopsy revealed small vessel neoformation, fibrosis and intraluminal thrombi, without vessel wall inflammation. He took voluntary discharge and later died apparently, due to diffuse alveolar haemorrhage.Author comment: "However, we present the first two known cases of vasculitis associated with the use of probable levamisole-adulterated cocaine in our country, severely affected by increasing drug traffic and consumption, with a peculiar histopathologic finding."Flores-Suarez LF, et al.
