Thrombotic thrombocytopenic purpura in pulmonary-renal syndromes

Abstract: Thrombotic thrombocytopenic purpura pathologically consists of a thrombotic microangiopathy that classically spares lung tissues. We describe a case of TTP that presented as a pulmonary-renal syndrome. In reviewing the international literature, pulmonary involvement is not as rare as once was thought, and the diagnosis of TTP should be considered in the differential diagnosis of pulmonary-renal syndromes. Goodpasture syndrome or positive GBM has been reported in several CVD cases (17)(18)(19)(20) (1,3,(21)(22)(23)(24)(25). Recently, in contrast to that deficient ADAMTS13 activity causes idiopathic or hereditary TTP, endothelial involvement has been shown to contribute to TTP pathogenesis in CVD patients (26,27 …”

“…The occurrence of a DAH is an unusual feature in the setting of TMA. However, the possible presence of acute pulmonary manifestations in TTP/HUS has been already mentioned (12)(13)(14)(15)(16)(17)(18)(19)(20). In those cases with acute respiratory symptoms, the issue was most often fatal and the condition associated with TTP/HUS were mostly cancer and/or its chemotherapy and less often bone marrow transplantation, clopidogrel use, or idiopathic TTP.…”

“…21 Myocardial failure and non-bacterial thrombotic endocarditis have also been described. [22][23][24] The reasons for the high incidence of TM observed in our study are unclear. None of the cytotoxics used in our 535 Table 2 Outcome after phases III to V See text for case report.…”
