Thrombotic Management of Antiphospholipid Syndrome: Towards Novel Targeted Therapies

Abstract: Antiphospholipid syndrome (APS) is a systemic autoimmune disease characterized by thrombosis and/or pregnancy morbidity with persistent levels of antiphospholipid antibodies (aPLs). The development of thrombosis in APS is mediated by aPLs and contributes to the high mortality rate in APS patients. However, although APS has been reported for more than 30 years, there has been no optimal regimen for its prevention or for the management of thrombosis, mainly because the mainstay treatment strategies for managing … Show more

“…In addition, several anecdotic reports have proposed beneficial effects of immunomodulatory agents including corticosteroids, rituximab, IvIg, D3 vitamin, plasmapheresis and chloroquine, anti-complement antibody eculizumab and the mTOR inhibitor rapamycin ( 26 , 40 , 41 ).…”

“…However, in many cases, the development of idiopathic DVT is caused by prolonged immobility leading directly to reduced blood flow predisposing to thrombus formation [ 42 ]. Some medications may change the subtle balance between coagulation and fibrinolysis, thus predisposing to thrombosis, for example, estrogens are directly linked to DVT and strokes [ 43 , 44 ]. From a mechanistic point of view, blood clot formation results from coagulation cascade activation that finally leads to fibrin generation.…”

“…4 The formation of thrombi both in arteries and veins is the major cause of mortality and morbidity associated with APS. 5 Antiphospholipid syndrome is an autoimmune procoagulant state mediated by aPLs, but a trigger is needed to precipitate thrombosis. This trigger can be in the form of infection, endothelial injury and surgery, leading to immobility or oral contraceptives.…”
