Threefold Increase in PrEP Uptake Over Time with High Adherence Among Young Men Who Have Sex With Men in Chicago

Abstract: The goal of this work is to better understand utilization and uptake of pre-exposure prophylaxis (PrEP) among young men who have sex with men (YMSM) and transgender women (TGW). We assessed trends and correlates of PrEP use and adherence across three time points of longitudinal data collection among 885 YMSM and TGW (aged 16-29) from the RADAR cohort in Chicago, 2015-2017. Past 6-month PrEP use increased across three visits: from 6.6 to 17.5%. In multivariable models, past 6-month PrEP use was significantly as… Show more

“…These data represent the largest published sample ever used to evaluate the PrEP continuum among MSM in the United States and include data from MSM in rural areas and smaller cities (critical because the use of many prevention services is lower among rural MSM ), and men recruited through non‐sex‐seeking online venues. Our data extend the findings of the NHBS , and previous online studies recruited exclusively from online sex‐seeking websites or in specific US cities . Although these data are from a convenience sample and therefore have limited external generalizability, it is critical to triangulate data on PrEP use from multiple sources, in light of their limitations.…”

“…With respect to the uptake rate of PrEP in our study, our findings are consistent with results documented in other settings in which PrEP uptake ranged between 60% and 93% [22][23][24] among MSM reporting condomless RAI [23] or condomless anal sex [25]. Although we did not find significant differences in PrEP eligibility at baseline compared to the end of the follow-up period in our study, four men became eligible during follow-up, highlighting the importance of period reassessments of HIV risk.…”

“…We previously identified lack of health insurance as a risk factor for acquiring HIV infection among black MSM in Atlanta , and those who choose to initiate PrEP have a higher prevalence of sexual risk behaviours . Thus, our finding that PrEP‐interested YBMSM without insurance have the longest delays between receipt of a PrEP prescription and PrEP initiation is especially concerning.…”
